Monotherapy with stenting in subarachnoid hemorrhage (SAH) after middle cerebral artery dissection
We describe a young patient who presented with acute headache from a subarachnoid hemorrhage that was ultimately determined to be due to a vascular dissection in the middle cerebral artery. The initial angiogram showed vascular irregularities in this area with stenosis. Repeat imaging 4 days after presentation identified a pseudoaneurysm proximal to the stenosis. The patient was successfully treated with a self-expanding nitinol stent and followed up with serial angiography during postoperative recovery in the hospital; additional angiograms were performed approximately 1 and 6 months after treatment. O-007 german thrombectomy database from 2012 to 2014: experiences from 9,365 acute stroke interventions
DiscussionRecanalization rates, functional outcomes and the rate of intraprocedural hemorrhages are in line with the data of the recently published RCTs. The absolute numbers need to be considered with caution as self-reported rates are usually more optimistic than evaluations by independent adjudicators. During the observation period there were no clear trends towards better outcomes or decreasing rates of rare complications. Acute carotid stenting for treatment of stuttering transient ischemic attacks after recent carotid endarterectomy
Carotid endarterectomy (CEA) is the established standard to treat occlusive extracranial atherosclerotic carotid disease. Complications of CEA must be recognized and dealt with efficiently due to the potentially catastrophic neurologic sequelae. A 67-year-old African American man was transferred from an outside hospital for an acute stroke.
