P151/175 Direct aspiration with EMBOVAC - first clinical experience and clot composition (results the "PERFECT study)
ConclusionThis is the first clinical evaluation of EMBOVAC. Direct aspiration with EMBOVAC as first line therapy resulted in high rates of reperfusion and favorable clinical outcomes. Clot composition analysis can aid in better understanding of the stroke etiology and means for effective clot removal.Disclosure of InterestAcandis (OJ, CL, JF); Microvention (GM, CC, JF); Stryker (GM, CC, JF); Medtronic (GM, CC, JF); Surge2surgery (BG); MIVI (CC); Phenox (CL, JF); Penumbra (CL, JF); GSK (AS); German Ministry of Science & Education and Economy & Innovation, EU, Hamburgische Investitions-/Förderbank, Philips, B...

P64 NIMBUS geometric clot extractor for tough clots: SPERO study results and clot composition
ConclusionsIn challenging cases with tough clot, first pass of NIMBUS retrieved clot richer in fibrin and lower in RBC content than clot retrieved with the first pass of standard MT devices.ReferencesFennell VS, Setlur Nagesh SV, Meess KM, Gutierrez L, James RH, Springer ME, Siddiqui AH. EP44* Nimbus geometric clot extractor for tough clots - Real world clinical experience and clot composition
ConclusionsNimbus is effective in removing tough clots markedly rich in fibrin and platelets in challenging real world situations.ReferenceJens Fiehler. Failed thrombectomy in acute ischemic stroke: return of the stent? Stroke 2018;49:811–812. https://doi.org/10.1161/STROKEAHA.118.020541DisclosureThis series has emanated from research supported in part by a grant from Science Foundation Ireland (SFI) and the European Regional Development Fund (ERDF) under grant number 13/RC/2073_P2. It is the independent clinical practice of the authors. Cerenovus provided support for post hoc data collection and analysis.

EP47* Investigation of current and novel treatment strategies for ICA and MCA occlusions in an in-vitro thrombectomy model
ConclusionsThe novel 0.088’’ aspiration catheter achieves the best FPE rates in both ICA and MCA occlusions. In terms of existing techniques, ADAPT is more effective in the MCA, while the stentrievers provide a benefit in the ICA.ReferencesHafeez MU, Kan P, Srivatsan A, Moore S, Jafari M, DeLaGarza C, et al. EP51* Burden of tough to retrieve clots in acute ischemic stroke: clinical and economic impact of increasing retrieval attempts in mechanical thrombectomy
ConclusionOdds of achieving good outcomes decline with MT passes, while care costs increase. Thrombectomy devices that improve interaction with tough clots for rapid and complete retrieval in fewer passes may improve clinical and economic outcomes.ReferencesYoo AJ.
