Hyperglycemic control in acute ischemic stroke patients undergoing endovascular treatment: post hoc analysis of the Stroke Hyperglycemia Insulin Network Effort trial
Conclusion We did not identify any beneficial effect of intensive glucose reduction on rates of death or favorable outcomes at 90 days among acute ischemic stroke patients undergoing endovascular treatment.

O-042 Incidence of periprocedural stroke with radial access for diagnostic cerebral angiograms
ConclusionIn our experience, transradial access for diagnostic cerebral angiograms was associated with a low but not neglectable incidence of periprocedural strokes when compared to the traditional femoral approach. Poor clinical outcome despite successful basilar occlusion recanalization in the early time window: incidence and predictors
Conclusions Early successful reperfusion with EVT occurred in 80.5% of patients, and the only treatment-related factor predictive of clinical outcome was first pass mTICI 2b-3 reperfusion. Further research is warranted to identify the optimal techniques and devices associated with first pass reperfusion in the posterior circulation.

E-176 Endovascular stent retriever thrombectomy of medium and small vessel occlusion of the middle cerebral artery with acute ischemic stroke: a case series
ConclusionsOur case series demonstrated that SRMT in SVO with disabling NIHSS is feasible and safe if performed carefully. All patient achieved TICI 3 perfusion and first pass effect was better observed in trap technique and use of long solitaire STD. Good function outcomes were achieved in all cases irrespective of the types of SRD used and passes required to achieve TICI 3 perfusion. Further studies are required.Disclosures Y. Lodi: None.

Carotid artery direct access for mechanical thrombectomy: the Carotid Artery Puncture Evaluation (CARE) study
Conclusion In this large multicentric study, DCP was scantly performed for vascular access to perform MT (0.39%) in patients with AIS-LVO and had a high rate of complications (18.5%). Our results provide arguments for not closing the cervical access by manual compression after MT.

Thrombectomy in basilar artery occlusions: impact of number of passes and futile reperfusion
Conclusions We found that BAO patients had a significant relationship between the number of MT passes and both recanalization and favorable functional outcome. We further found that the benefit of recanalization in BAO patients was significant only when recanalization was achieved within three passes, encouraging at least three passes before stopping the procedure.

E-114 Rescue suction mechanical thrombectomy of medium-small vessel occlusion in a pediatric patient presented with acute ischemic stroke
ConclusionEndovascular aspiration mechanical thrombectomy of MSVO is feasible in pediatric patients presenting with AIS and associated with good functional outcome. Additionally, when acute ongoing neurological deficit is present and CTA is inconclusive, a digital subtraction angiography is deemed needed to identify MSVO as the yields of CTA diminishes significantly beyond LVO. Mechanical thrombectomy is cost-effective versus medical management alone around Europe in patients with low ASPECTS
Conclusions MT is efficient versus MM alone for patients with low ASPECTS in eight countries across Europe. Patients with a large ischemic core could be treated with MT because it is both clinically beneficial and economically sustainable.

Use of a sheathless 8-French balloon guide catheter (Walrus) through the radial artery for mechanical thrombectomy: technique and case series
Conclusions The use of this sheathless catheter for transradial access was safe and feasible. The technique can potentially improve the outcomes of transradial access for stroke intervention.
