Abstract 223: National and Regional Trends in Hospitalizations for Hemorrhagic Stroke after Acute Myocardial Infarction in the United States: 1999-2010 Poster Session II
Conclusions: From 1999 to 2010, the overall hospitalization rates of hemorrhagic stroke after AMI were relatively stable without significant changes across all subgroups. Thirty-day mortality rates remained largely unchanged over time. Stroke risk in the stroke belt was not found significantly higher comparing with non-stroke belt states.

Abstract 233: Trends in Referral to Post-Acute Care Services after an Acute Stroke Hospitalization in the U.S.: Findings from the Get With The Guidelines-Stroke Program Poster Session II
Conclusions: Since 2003 there was a slight increase in the proportion of stroke patients discharged to post-acute care among hospitals participating in GWTG-Stroke. However, 4 in 10 patients are discharged home without services. Over the last decade of payment reform, there has been modest change in types but not the overall referral to post-acute services for stroke patients.

Abstract 234: Prior Warfarin Treatment and Intracranial Hemorrhage among Patients With Acute Ischemic Stroke Treated With Intravenous Tissue Plasminogen Activator: A Meta-analysis Poster Session II
Conclusions: These data suggested that the risk of sICH after thrombolytic therapy is not increased in patients using warfarin with sub therapeutic INR levels.

Abstract 235: Acute Care Measures of Rehabilitation Needs Associated with Discharge to Rehabilitation for Patients with Minor Stroke Poster Session II
Conclusions: Rehabilitation needs were documented for patients with minor stroke and almost one third were discharged to receive inpatient rehabilitation in an IRF or SNF. Taking pre-stroke disability into consideration for transition planning from acute care could facilitate earlier communication and planning with the next point of care.

Abstract 238: Utilization and Safety of a Heparin Nomogram in Treating Thrombotic Comorbidities in Stroke Patients Poster Session II
CONCLUSION: The heparin stroke nomogram appears safe in that it does not increase risk of ICH. However, therapeutic aPTTs are not reached quickly with less than half of patients therapeutic at 24 hours, nor are they maintained at steady state. This nomogram could be too conservative in cases where clinical situation requires rapid anticoagulation.

Abstract 243: Impact of Dual Health Care System Use on Quality of Warfarin Anticoagulation in Atrial Fibrillation Poster Session II
Conclusion: More than half of Veterans age ≥ 65 with AF and VA-issued warfarin receive outpatient INR testing in MC. Dual health care system use is associated with lower achievement of process measures of quality of anticoagulation care, which could affect outcomes of stroke, hemorrhage and death.

Abstract 246: Reversible Cerebral Vasoconstriction Syndrome and Spontaneous Hemorrhage are the Most Common Causes of Peripartum Subarachnoid and Intracerebral Hemorrhage. Poster Session II
Conclusions: Etiologies in peripartum SAH and ICH differ from the general population. Aneurysmal bleeds were absent in our case series. Rates of SAH and ICH may be higher in patients who underwent cesarean section. In conclusion, peripartum subarachnoid and intracranial hemorrhages have a different pathophysiologic process than in the general population.

Abstract 250: Economic Burden of Mortality and Cardiovascular Events among Patients with Acute Coronary Syndromes in a Commercial Health Plan Poster Session III
Conclusion: Our findings suggest that a modest 10% increase in anticoagulant use among patients with ACS would reduce mortality, MI, ST and related healthcare costs by 4%, 0.7%, and 3%, respectively. Addition of anticoagulation therapy potentially reduces the incidence of ACS-related mortality, MI, ST and associated healthcare costs to a commercial health plan, and benefits from anticoagulation use should be balanced against the risk of bleeding.
