Relative Prognostic Importance and Optimal Levels of Risk Factors for Mortality and Cardiovascular Outcomes in Type 1 Diabetes.
CONCLUSIONS: Glycated hemoglobin, albuminuria, duration of diabetes, systolic blood pressure and low-density lipoprotein cholesterol appear the most important predictors for mortality and cardiovascular outcomes in patients with type 1 diabetes. Lower levels for glycated hemoglobin, systolic blood pressure and low-density lipoprotein cholesterol than contemporary guideline target levels appear to be associated with significantly lower risk for outcomes. Angiopoietin-2 Inhibition Rescues Arteriovenous Malformation in a Smad4 Hereditary Hemorrhagic Telangiectasia Mouse Model.
CONCLUSIONS: Our studies provide a novel mechanism whereby loss of Smad4 causes increased Angpt2 transcription in ECs leading to AVM formation, increased blood vessel calibers and changes in EC morphology in the retina. Blockade of ANGPT2 function in an in vivo Smad4 model of HHT alleviated these vascular phenotypes further implicating ANGPT2 as an important TGFβ downstream mediator of AVM formation. Therefore, alternative approaches that target ANGPT2 function may have therapeutic value for the alleviation of HHT symptoms, such as AVMs. PMID: 30744395 [PubMed - as supplied by publisher]

Use, Temporal Trends, and Outcomes of Endovascular Therapy after Interhospital Transfer in the United States.
CONCLUSIONS: Interhospital transfer for EVT is increasingly common and is associated with a significant delay in EVT initiation highlighting the need to develop more efficient stroke systems of care. Assessing the causal role of body mass index on cardiovascular health in young adults: Mendelian randomization and recall-by-genotype analyses.
Conclusions: Complementary MR and RbG causal methodologies, together with a range of sensitivity analyses, suggest that higher BMI is likely to cause worse cardiovascular health, specifically higher BP and LVMI, even in youth. Higher BMI also resulted in increased CO in the RbG study, which appeared to be solely driven by SV, as neither MR nor RbG analyses suggested a causal effect of BMI on heart rate. These consistent results support efforts to reduce BMI from a young age to prevent later adverse cardiovascular health and illustrate the potential for phenotypic resolution with maintained analytical power using RbG. ...

Multiple Arterial Grafting Is Associated With Better Outcomes for Coronary Artery Bypass Grafting Patients.

Higher Aortic Stiffness is Related to Lower Cerebral Blood Flow and Preserved Cerebrovascular Reactivity in Older Adults.
Conclusions -Greater aortic stiffening relates to lower regional CBF and higher CVR in cognitively normal older adults, especially among individuals with increased genetic predisposition for Alzheimer's disease. Heart Failure After Ischemic Stroke or TIA in Insulin-Resistant Patients Without Diabetes Treated with Pioglitazone.
Conclusions -In IRIS, with surveillance and dose adjustments, pioglitazone did not increase risk of HF, and conferred net cardiovascular benefit in patients with insulin resistance and cerebrovascular disease. The risk of HF with pioglitazone was not modified by baseline HF risk. The IRIS experience may be instructive for maximizing the net benefit of this therapy. Assessment of Cardiovascular Risk in Older Patients with Gout Initiating Febuxostat versus Allopurinol: A Population-Based Cohort Study.
Conclusions -Among a cohort of 99,744 older Medicare patients with gout, overall there was no difference in the risk of MI, stroke, new onset HF, coronary revascularization, or all-cause mortality between patients initiating febuxostat compared with allopurinol. However, there seemed to be a trend toward an increased, albeit not statistically significant, risk for all-cause mortality in patients who used febuxostat for over 3 years versus allopurinol for over 3 years. Periprocedural Outcomes of Direct Oral Anticoagulants vs. Warfarin in Non-Valvular Atrial Fibrillation: A Meta-analysis of Phase III Trials.
