Canadian Stroke Best Practice Recommendations: Mood, Cognition and Fatigue Following Stroke practice guidelines, update 2015
Every year, approximately 62 000 people with stroke and transient ischemic attack are treated in Canadian hospitals, and the evidence suggests one‐third or more will experience vascular‐cognitive impairment, and/or intractable fatigue, either alone or in combination. The 2015 update of the Canadian Stroke Best Practice Recommendations: Mood, Cognition and Fatigue Module guideline is a comprehensive summary of current evidence‐based recommendations for clinicians in a range of settings, who provide care to patients following stroke. The three consequences of stroke that are the focus of the this guideline (poststrok...

Implications of stroke for caregiver outcomes: findings from the ASPIRE‐S study
ConclusionFindings indicate that caregiver and stroke survivor well‐being are interdependent. Thus, early interventions, including increased training and support programs that include caregivers, are likely to reduce the risk of negative emotional outcomes.

TALOS: a multicenter, randomized, double‐blind, placebo‐controlled trial to test the effects of citalopram in patients with acute stroke
DiscussionSSRI treatment is well tolerated and overall beneficial in the wake of stroke; it may also be neuroprotective and prevent new vascular events.

ICARUSS, the Integrated Care for the Reduction of Secondary Stroke trial: rationale and design of a randomized controlled trial of a multimodal intervention to prevent recurrent stroke in patients with a recent cerebrovascular event, ACTRN = 12611000264987
ConclusionThe ICARUSS study aims to recruit and follow up patients between 2007 and 2013 and demonstrate the effectiveness of exposure to the ICARUSS model in stroke survivors to reduce recurrent stroke or vascular events and promote the implementation of best practice risk factor management at primary care level.

Circadian sleep/wake rhythm abnormalities as a risk factor of a poststroke apathy
ConclusionThese results indicate that early poststroke alterations of sleep/wake circadian rhythms – easily evaluated by actigraphy – are associated with a higher risk of poststroke apathy at three‐months. In terms of clinical outcomes, our results provide targets for very early identification of patients at risk to develop apathy after stroke and for assessing when to start specific therapy to optimize rehabilitation efficiency.

Poststroke fatigue: risk factors and its effect on functional status and health‐related quality of life
ConclusionDepressive symptoms and prestroke fatigue are major correlates of poststroke fatigue while more severe poststroke fatigue is associated with poorer activities of daily living and health‐related quality of life.

Part II: predictors of depression after stroke and impact of depression on stroke outcome: an updated systematic review of observational studies
ConclusionsEarly identification of patients at high risk of depression after stroke, those with a history of depression and physical disability after stroke, would enable the early implementation of effective management and prevention strategies for depression. The reciprocal relationship between depression and physical disability highlights the need for interventions that reduce disability after stroke, which may in turn improve mood and overall recovery for an increasingly large number of stroke survivors.

Part I: frequency of depression after stroke: an updated systematic review and meta‐analysis of observational studies
ConclusionDespite systematic review evidence describing validated depression screening tools and effective treatment and prevention strategies for depression after stroke, there has not been a significant reduction in the proportion of people experiencing depression after stroke. There is a pressing need for increased clinical uptake of evidenced‐based strategies to screen for, prevent, and treat depression after stroke.

Quality of life after surgical decompression for space‐occupying middle cerebral artery infarction: Systematic review
ConclusionsMost patients treated with surgical decompression for space‐occupying infarction have a reasonable quality of life at long‐term follow‐up and are satisfied with the treatment received. Severe depressive symptoms are uncommon.

STroke imAging pRevention and treatment (START): A longitudinal stroke cohort study: Clinical trials protocol
RationaleStroke and poststroke depression are common and have a profound and ongoing impact on an individual's quality of life. However, reliable biological correlates of poststroke depression and functional outcome have not been well established in humans. AimsOur aim is to identify biological factors, molecular and imaging, associated with poststroke depression and recovery that may be used to guide more targeted interventions. DesignIn a longitudinal cohort study of 200 stroke survivors, the START – STroke imAging pRevention and Treatment cohort, we will examine the relationship between gene expression, regulator prot...

Poststroke anxiety is prevalent at the population level, especially among socially deprived and younger age community stroke survivors
ConclusionUsing a conservative HADS‐A cutoff, a high prevalence of definite abnormal anxiety in community stroke survivors is observed. This prevalence increases markedly in younger and more socially deprived stroke survivors.

Depression: Cognition relations after stroke
ConclusionsThe rates of depression and cognitive impairment were in the range of previous stroke studies. Our study suggested a strong relation between depression and cognition characterized by executive functions and working memory dysfunctioning.

Effects of carotid endarterectomy or stenting on hemodynamic complications in the International Carotid Stenting Study: a randomized comparison
ConclusionHemodynamic depression occurs more often after carotid artery stenting and severe hypertension more often after carotid endarterectomy, but these complications are not responsible for the excess of major perioperative events after carotid artery stenting.
