A multicenter, randomized, double‐blinded, placebo‐controlled phase III study of Clot Lysis Evaluation of Accelerated Resolution of Intraventricular Hemorrhage (CLEAR III)
BackgroundIn adults, intraventricular thrombolytic therapy with recombinant tissue plasminogen activator (rtPA) facilitates resolution of intraventricular haemorrhage (IVH), reduces intracranial pressure, decreases duration of cerebrospinal fluid diversion, and may ameliorate direct neural injury. We hypothesize that patients with small parenchymal haematoma volumes (<30 cc) and relatively large IVH causing acute obstructive hydrocephalus would have improved clinical outcomes when given injections of low‐dose rtPA to accelerate lysis and evacuation of IVH compared with placebo. Effect of addition of clopidogrel to aspirin on stroke incidence: Meta‐analysis of randomized trials
ConclusionsThis meta‐analysis demonstrates a substantial relative risk reduction in stroke by clopidogrel plus aspirin vs. aspirin alone that is consistent across different trial cohorts. Major haemorrhage is increased by dual antiplatelet therapy.

Cerebral CT perfusion in patients with perimesencephalic and those with aneurysmal subarachnoid hemorrhage
ConclusionsPerimesencephalic hemorrhage patients have a higher cerebral blood flow than aneurysmal subarachnoid hemorrhage patients. The findings of this study further support a venous origin of bleeding in perimesencephalic hemorrhage patients. Future studies should further elaborate on cerebral blood flow in posterior circulation aneurysms.

Dual antiplatelets reduce microembolic signals in patients with transient ischemic attack and minor stroke: subgroup analysis of CLAIR study
ConclusionsEarly dual therapy with clopidogrel and aspirin reduces microembolic signals in patients with minor ischemic stroke or transient ischemic attack, without causing significant bleeding complications.

This article emphasizes new observations on transient focal neurological episodes in the context of cerebral amyloid angiopathy, a common but under‐recognized small vessel disease most often encountered as a cause of symptomatic lobar intracerebral hemorrhage.
