Dextromethorphan/Quinidine for Pseudobulbar Affect Following Stroke: Safety and Effectiveness in the PRISM II Trial
Dextromethorphan (DM)/quinidine (Q) was approved for pseudobulbar affect (PBA) treatment based upon efficacy and safety trials in patients with PBA secondary to amyotrophic lateral sclerosis or multiple sclerosis. The PRISM II trial evaluated DM/Q as PBA treatment in patients with stroke, dementia or traumatic brain injury.

Efforts to augment post-stroke upper limb (UL) motor improvement include the use of newer interventions such as noninvasive brain stimulation (NIBS) and task practice in virtual reality environments (VEs). Despite increasing interest in using a combination of these 2 interventions, the effectiveness of this combination to enhance UL motor improvement outcomes has not been examined.

The Effect of Neural Lesion Type on Botulinum Toxin Dosage: A Retrospective Chart Review
Conclusion: In our practice, we found that treating spasticity in people with MS required the greatest dose of botulinum toxin, followed by CP and then stroke.

The Analgesic Effect of Abobotulinum and Incobotulinum Toxins Type A in Central Poststroke Pain: Two Case Reports
The International Association for the Study of Pain defines central pain as “pain initiated or caused by primary lesion or dysfunction of the central nervous system” [1]. If it results from a stroke, it is referred to as central poststroke pain (CPSP) [1]. CPSP prevalence reports vary from 8% to 46% [3-5]. This wide variation is possibly related to the heterogeneity of stroke lesions between the populations surveyed, differences in study design, and different timing of evaluations after stroke [6].

Demystifying Poststroke Pain: From Etiology to Treatment
Pain after stroke is commonly reported but often incompletely managed, which prevents optimal recovery. This situation occurs in part because of the esoteric nature of poststroke pain and its limited presence in current discussions of stroke management. The major specific afflictions that affect patients with stroke who experience pain include central poststroke pain, complex regional pain syndrome, and pain associated with spasticity and shoulder subluxation. Each disorder carries its own intricacies that require specific approaches to treatment and understanding.

A Localized Brain Lesion Resulting in Isolated Hand Weakness
Hand weakness is a common concern of patients who are seen by a physiatrist, either in an inpatient or outpatient setting. The differential diagnosis of isolated hand weakness is broad and includes trauma; musculoskeletal injury; arthritis; overuse syndromes, for example, carpal tunnel; peripheral neuropathy; radiculopathy; and central nervous system disorders. There is limited literature regarding stroke as a diagnosis for isolated intrinsic hand weakness, but, for patients with risk factors, stroke should be considered in the differential diagnosis [1].

Improvement in Compensation for Chronic Poststroke Homonymous Hemianopsia After Initiation of a Selective Serotonin Reuptake Inhibitor: A Case Report
Homonymous hemianopsia (HH) is a common adverse outcome after stroke. Spontaneous improvement more than 6 months poststroke is thought to be unlikely, and traditional visual rehabilitation techniques lack clear evidence of efficacy. The case presented is of a 22-year-old woman who demonstrated improved compensation of her stroke-induced HH after the initiation of a selective serotonin reuptake inhibitor (SSRI). There is evidence supporting the use of SSRIs to improve poststroke cognitive impairment, motor impairment, and depression.

The Original Description of Anton Syndrome
We would like to commend Eby et al [] regarding their recent case description in “The Rehabilitation of Anton Syndrome” and would like to share our perspective related to a review of Anton's original description. Many authors have used the eponym “Anton syndrome” to describe cortical blindness with visual anosognosia. Specifically, Eby et al [] state that “Anton syndrome, also referred to as visual anosognosia, is a rare stroke syndrome that involves the bilateral occipital lobes seen most commonly as a result of cerebrovascular disease.” This description is consistent with many recent descriptions of this syndrome.
