Approaches in Prehospital Sepsis Screening
Discussion In the United States, sepsis is one of the leading causes of death, which requires timely identification and proper treatment (CDC, 2019; Guerra et al., 2020; Polito et al., 2015). Based upon a review of literature conducted from 2014 to 2018, the primary investigators could locate five prehospital EMS screening tools to assist EMS providers in identifying at-risk sepsis patients. The researchers explored how the modified SIRS and qSOFA scoring systems were used in hospital settings in addition to these five prehospital EMS screening tools. The Bas 90-30-90, Guerra, PRESEP, PRESS, and Robson tools have ove...

Whatcom County (WA) Seeks Better, Cheaper Answer to 911 Call Centers
Robert Mittendorf The Bellingham Herald (Bellingham, Wash.) (MCT) Dec. 15—Combining Whatcom County’s two 911 dispatch centers, whose operators separately send law-enforcement officers or firefighters to calls for service, is back on the County Council’s agenda. It’s both logical and a cost-saving measure to combine the two 911 centers, said Councilman Tyler Byrd. “I’ve been working on all the documentation I can find on this and talking to people. I can’t find a single efficiency that comes out of this the way it is today,” Byrd said during a No...

Managing NEMSIS Transitions Requires Simplicity, Training & Extensive Planning
“How do we know we’re doing well? What should we be improving?” Anyone involved in the delivery of EMS asks these questions. They lead to more detailed questions like: “What’s our time to STEMI alert? Stroke alert? Taking a 12-lead? What indicators should guide us to transport to a Level 1 (or Level 2, 3, 4) trauma center?” We also continually ask, “And are we in need of improvement in any area?” Answering these questions requires data—on your own performance and  on what other agencies are doing. It’s that comparative data that sheds light on areas of improvement. Since about 2003, 52 U.S. states ...

BOZEMAN, MT -- Stroke is a leading cause of disability and death in the U.S., with close to 800,000 cases each year. Improving Systems of Care in Time-Sensitive Emergencies
This report paved the way for modern EMS by recommending the creation of standards for ambulance services, training and oversight in prehospital medicine.1 Legendary physician R Adams Cowley is credited with creating and popularizing the concept of a "golden hour" in trauma, underscoring the need for coordination from the moment trauma occurs to the time to definitive care. Deficiencies in trauma care were recognized and trauma systems of care were developed with EMS playing a central role. As EMS evolved, so did the need to include EMS when developing systems of care for other time-sensitive emergencies. Cardiov...
