Left ventricular thrombus after ST segment elevation myocardial infarction: a single-centre observational study.
CONCLUSIONS: The incidence of LV thrombus following STEMI was low and it was associated with a low rate of stroke and systemic embolism but high mortality. Randomised studies are needed to evaluate the efficacy of NOAC's in this context. PMID: 33332339 [PubMed - in process]

New Zealand hospital stroke service provision.
CONCLUSION: Hospital location is associated with differences in stroke services provision across New Zealand and ongoing work is required to optimise consistent access to best practice care. These results, in conjunction with an ongoing (REGIONS Care) study, will be used to determine whether this affects patient outcomes. PMID: 33332337 [PubMed - in process]

Atrial fibrillation in acute coronary syndrome: patient characteristics and appropriate utilisation of anti-thrombotic therapy in New Zealand (ANZACS-QI 39).
CONCLUSION: In New Zealand, one in seven patients presenting with ACS have AF, a third being new-onset AF. Antithrombotic management is inconsistent, with underutilisation of anticoagulants, particularly the DAT regimen, and is inadequately informed by stroke and bleeding risk scores. Bilateral vertebral artery dissection and cerebellar stroke: a rare complication of massage.
We describe the case of a 39-year-old male, who presented to our emergency department after a one-day history of headache and vomiting, with associated sudden onset posterior neck pain and cerebellar signs following a massage. Computed tomography angiogram and brain demonstrated bilateral vertebral artery dissection and cerebellar stroke. He was admitted to hospital for monitoring and conservative management with antiplatelet therapy, resulting in a good outcome. This is the first reported case of bilateral vertebral artery dissection and stroke to be associated with massage. This case also suggests, unlike many reports in...

Trial removal of indwelling urinary catheters in stroke patients: a clinical audit at North Shore Hospital.
CONCLUSIONS: Insufficient documentation and inappropriate indications for IDC insertion were features noted during this audit. Daily electronic reminders and prompting concerning TROC are important to reduce catheter days and reduce infection rates. Indwelling catheters and associated infections impact the length of hospitalisation, mortality and morbidity of stroke patients. PMID: 32242174 [PubMed - in process]

Impact of the national public 'FAST' campaigns.
CONCLUSION: The New Zealand stroke public campaigns were associated with a rise in stroke symptom recognition, time-critical awareness, ambulance stroke notifications and thrombolysis rates. The uncontrolled nature of this study necessitates consideration of other potential contributing factors when interpreting results. Ongoing campaigns for continual reinforcement appear important. Incidence of stroke and traumatic brain injury in New Zealand: contrasting the BIONIC and ARCOS-IV studies.
CONCLUSIONS: Differences in TBI and stroke incidence suggest targeting prevention very differently for the two groups. Incidence profiles suggest TBI is much more common; and a need to target males and those of Māori ethnicity for TBI prevention. PMID: 31563926 [PubMed - in process]

Mitral valve surgery with or without coronary bypass grafting: eight-year cohort study.
CONCLUSION: MVR+CABG compared to MVR patients had more comorbidities and greater operative risk, and were independently associated with higher operative mortality and composite morbidity, but not independently associated with higher long-term mortality. PMID: 31415499 [PubMed - in process]

How much rehabilitation are our patients with stroke receiving?
CONCLUSION: There were delays in providing an initial community rehabilitation appointment and insufficient therapy intensity when comparing audit results to New Zealand Guideline expectations. As a result of this audit, recommendations for service improvements have been made. PMID: 31352474 [PubMed - in process]

Endovascular clot retrieval for acute ischaemic stroke in New Zealand.
CONCLUSIONS: This study has shown that stroke endovascular clot retrieval is being provided safely and effectively in New Zealand. However, there remain discrepancies in service provision, and ongoing regional, inter-regional and inter-sector collaboration is essential to implement comprehensive and equitable ECR services across the country.
