Successful manual thrombus aspiration in anterior ST-segment elevation myocardial infarction due to cardioembolic obstruction of the left main coronary artery
Publication date: Available online 5 December 2019Source: Journal of Cardiology CasesAuthor(s): Diego Della Riva, Matteo Bruno, Nevio TaglieriAbstractLarge clinical trials and meta-analyses have shown that thrombus aspiration (TA) in the setting of ST-T segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PPCI) does not improve clinical outcome, whilst it may be associated with an increased risk of stroke. Accordingly, in the most recent European Society of Cardiology guidelines the role of routine TA during PPCI has been downgraded to a class III recommendation with level ...

Pulmonary embolism with migrating thrombus through patent foramen ovale: A case for a mixed pharmacological and percutaneous management
Publication date: Available online 27 September 2018Source: Journal of Cardiology CasesAuthor(s): Gloria Santangelo, Alfonso Ielasi, Francesco Pattarino, Antonio Tommaso Saino, Pasquale Antonio Scopelliti, Maurizio TespiliAbstractA 61-year-old man, admitted to our hospital for bilateral pulmonary embolism, complicated by right renal ischemia and multiple splenic infarcts due to a mobile thrombus entrapped in a patent foramen ovale, has been successfully treated with apixaban 5 mg twice daily followed by transcatheter patent foramen ovale closure.<Learning objective: Apixaban could be a therapeutic alternative to t...

Paradoxical embolism caused by ovarian vein thrombosis extending to inferior vena cava in a female with uterine myoma
We report the case of a 42-year-old female who was hospitalized for stroke. Detailed investigations revealed the existence of a PFO, pulmonary embolism, and ovarian vein thrombosis extending to inferior vena cava. She had a uterine myoma to be operated on but no other thrombophilic disorders. Anticoagulation therapy with direct oral anticoagulant successfully reduced the thrombus and prevented the recurrence of paradoxical embolism.<Learning objective: Ovarian vein thrombosis (OVT) is often associated with malignancy, pelvic surgery, or pregnancy. This case shows uterine myoma as a possible cause of OVT and demonstrate...

Long-standing persistent effects of atrial volume reduction combined with pulmonary vein isolation
Publication date: Available online 18 June 2018Source: Journal of Cardiology CasesAuthor(s): Masami Takagaki, Naoko Ikeda, Hiroki Yamaguchi, Shinichi Mitsuyama, Tasuku Kadowaki, Tatsuya NakaoAbstractAlthough surgical left atrial (LA) volume reduction combined with mitral valve surgery and/or surgical ablation for atrial fibrillation has been reported to be effective, its long-term outcomes in the absence of mitral procedure are not well established. A 74-year-old man with two previous sternotomies—the first for pericardiectomy due to constrictive pericarditis and the second for mitral valve replacement with mechanical va...

Fourth mitral valve replacement in a 15-year span in a woman of childbearing age
Publication date: Available online 13 June 2018Source: Journal of Cardiology CasesAuthor(s): Laura Leticia Rodríguez-Chávez, Grecia Iveth Maryelis Raymundo-Martínez, Rodrigo Gopar-NietoAbstractThere is no report of four valve replacement surgeries in a woman during 15-year follow-up. A 23-year-old female was diagnosed at 7 years with rheumatic fever. She developed severe mitral regurgitation and at the same age a prosthetic valve was placed. In the next 15 years her mitral valve was changed 3 times because of clinical, echocardiographic, and histopathologic evidence of thrombosis. Her coagulation tests all showed infrat...

Percutaneous isolation of left atrial appendage thrombus
Publication date: Available online 16 June 2017 Source:Journal of Cardiology Cases Author(s): Oh-Hyun Lee, Jae-Sun Uhm, Jung-Sun Kim, In-Jeong Cho, Geu-Ru Hong, Hui-Nam Pak, Yangsoo Jang Most physicians regard left atrial appendage (LAA) thrombus as a contraindication for LAA occlusion due to risk of distal embolization which is a serious complication. Here we report a case of successfully implanted Amplazter cardiac plug without complication in elderly patients having LAA thrombus with recurrent embolic events despite oral anticoagulants for prevention of thromboembolic events. <Learning objective: LAA thrombus is...

Rivaroxaban for treatment of intraventricular thrombus in Chagas disease
We present a case of a 61-year-old man admitted for stroke 5 months after a renal embolism. An intraventricular thrombus was observed, probably the source of the cerebral and renal embolisms. The patient refused warfarin and rivaroxaban was used instead. After 40 days of treatment the thrombus had dissolved, after 20 months of regular use of rivaroxaban no more embolic events were observed. The use of rivaroxaban was effective in preventing embolic events in Chagas disease and intraventricular thrombus. <Learning objective: Warfarin is recommended for stroke prevention in patients with Chagas disease and left ventri...
