Differences in outcomes in patients with stable coronary artery disease managed by cardiologists versus non-cardiologists: the international prospective CLARIFY registry.
CONCLUSIONS    Outpatients with SCAD managed by cardiologists had a lower rate of cardiovascular outcomes than those managed by non-cardiologists. We did not find clear evidence that cardiologists provided superior guideline-based treatment, so the differences in outcome were most likely due to unquantifiable differences in patient characteristics. Assessment of the mean time in the therapeutic INR range and the SAME-TT2R2 score in patients with atrial fibrillation and cognitive impairment.
CONCLUSIONS    Cognitive disorders in patients with AF significantly reduce the efficacy of VKA therapy. The decision to administer VKA treatment should be based not only on the CHA2DS2-VASc and HAS-BLED scores, but also on the SAMe-TT2R2 score and the evaluation of the patient's cognitive functions. Management of patients with atrial fibrillation and chronic kidney disease in light of the latest guidelines.
Authors: Młodawska E, Tomaszuk-Kazberuk A, Łopatowska P, Musiał WJ, Małyszko J Abstract Atrial fibrillation (AF) frequently occurs in patients with chronic kidney disease (CKD), and the rate reaches even 30% in patients with end-stage renal disease (ESRD). Patients with AF and CKD have a significantly higher risk of thrombotic complications, particularly ischemic stroke, and at the same time, a higher bleeding risk (proportionally to the grade of renal failure). In addition, AF and CKD share a number of comorbidities and risk factors, which results in increased mortality rates. Moreover, disturbances in hemosta...

Patients with atrial fibrillation and low risk of stroke - do they really need anticoagulation?
Authors: Trusz-Gluza M, Filipecki A, Urbańczyk-Świć D Abstract Atrial fibrillation (AF) significantly increases risk of thromboembolic events, in particular the risk of stroke. Anticoagulation therapy reduces this risk and the treatment should be lifelong. However, the risk in patients with non-valvular AF is not equally distributed. There are patients at low risk. Assessment of the thromboembolic risk according to guidelines is mandatory, most frequently CHA2DS2-VASc scheme is used. In the large group with the CHA2DS2-VASc-0 (females 1) the annual risk of stroke was 0.49%, ischemic stroke 0.43%, on the other si...

Perioperative anticoagulant management in patients with atrial fibrillation: practical implications of recent clinical trials.
Authors: Tafur A, Douketis JD Abstract Defining the safest perioperative anticoagulation management approach for patients who are receiving chronic anticoagulant therapy stroke prevention has been a challenging and longstanding dilemma, especially for patients with atrial fibrillation who constitute the most common patient group receiving long term anticoagulation. Using a case-based format we summarize the findings of recent clinical trials which have helped to informed best practices for perioperative anticoagulant management in patients with atrial fibrillation and provide an algorithmic management approach to t...

Intraaortic balloon pump in patients with cardiogenic shock complicating myocardial infarction: a systematic review and meta-analysis of randomized trials.
CONCLUSIONS: The use of IABP in patients with cardiogenic shock complicating myocardial ischemia does not reduce mortality (moderate confidence) and is not associated with a higher risk of complications (very low to low confidence). The results should be interpreted with caution owing to limitations such as imprecision, risk of bias, and clinical heterogeneity. PMID: 25698226 [PubMed - in process]
