Resuscitation Strategies in Early Septic Shock: A Survey of Puerto Rico Intensive Care Physicians.
CONCLUSION: Significant variability exists in the management of ESS among physicians in the ICU in PR. Compared to other studies, fewer physicians in PR use invasive monitoring techniques. These results highlight the need for quality education and training in CCM as well as continuing education in the field. PMID: 30924909 [PubMed - in process]

Incomplete limb ischemia as a complication in a pediatric patient with toxic-shock syndrome
We present the case of a 3-year-old boy who developed a toxic shock syndrome with multiple organ failure after streptococcal A pharyngitis. In addition, the combination of an indwelling arterial catheter in the left femoral artery, high-dose catecholamine administration and desolate condition of the extremities in the course of the shock syndrome led to a pronounced dermal defect on the left distal lower leg with muscular necrosis. Modern intensive care medicine makes the survival of the most serious diseases possible. Invasive methods are indispensable. Unfavorable constellations, such as hypotension, coagulation disorder...

Drugs acting on the heart: heart failure and coronary insufficiency
Publication date: Available online 1 April 2015 Source:Anaesthesia & Intensive Care Medicine Author(s): Takashi Kudo , Jonathan P. Thompson Heart failure (HF) and coronary insufficiency are common amongst surgical and critical care patients. Both are chronic conditions interrupted by acute episodes. HF activates neurohormonal mechanisms that worsen renal and cardiac function. Acute heart failure commonly presents with dyspnoea as a consequence of systolic and/or diastolic dysfunction. Goals of treatment are symptom relief, to maintain tissue perfusion and optimize cardiac function. Diuretics and vasodilators are ...

Neuromonitoring
Publication date: Available online 7 April 2017 Source:Anaesthesia & Intensive Care Medicine Author(s): Malcolm E. Smith, Martin Smith Management of acute brain injury is based on a central concept that prevention of secondary hypoxic/ischaemic injury is associated with improved outcomes. While clinical assessment of neurological state remains fundamental to neuromonitoring, several techniques are available for global and regional brain monitoring that provide assessment of cerebral perfusion, oxygenation and metabolic status, and early warning of impending brain hypoxia/ischaemia. Developments in multimodality mo...
