Implementation of a primary care asthma management quality improvement programme across 68 general practice sites
NPJ Prim Care Respir Med. 2023 May 13;33(1):21. doi: 10.1038/s41533-023-00341-y.ABSTRACTDespite national and international guidelines, asthma is frequently misdiagnosed, control is poor and unnecessary deaths are far too common. Large scale asthma management programme such as that undertaken in Finland, can improve asthma outcomes. A primary care asthma management quality improvement programme was developed with the support of the British Lung Foundation (now Asthma + Lung UK) and Optimum Patient Care (OPC) Limited. It was delivered and cascaded to all relevant staff at participating practices in three Clinical Commissioni...

Care by general practitioners for patients with asthma or COPD during the COVID-19 pandemic
NPJ Prim Care Respir Med. 2023 Apr 8;33(1):15. doi: 10.1038/s41533-023-00340-z.ABSTRACTThe impact of the COVID-19 pandemic on general practitioners' (GP) care for patients with asthma and/or COPD is largely unknown. To describe the impact of the pandemic on asthma or COPD-related GP care, we analysed routinely recorded electronic health records data from Dutch general practices and out-of-hours (OOH) services. During the COVID-19 pandemic (2020), the contact rates for asthma and/or COPD were significantly lower in GP practices and OOH services compared with the pre-pandemic period (2019) (respectively, 15% lower and 28% lo...

Asthma medication adherence and exacerbations and lung function in children managed in Leicester primary care
NPJ Prim Care Respir Med. 2023 Mar 25;33(1):12. doi: 10.1038/s41533-022-00323-6.ABSTRACTPoor adherence to asthma preventer medication is associated with life-threatening asthma attacks. The quality and outcomes framework mandated primary care annual asthma review does not include adherence monitoring and the effect of poor adherence on lung function in paediatric primary care patients is unknown. The aim was to investigate the link between inhaled corticosteroid (ICS) adherence and spirometry, fraction of exhaled nitric oxide (FeNO) and asthma control in asthmatic school-age children in this cross-sectional observational s...

One-minute sit-to-stand test as a quick functional test for people with COPD in general practice
This study aimed to identify a quick test to measure functional exercise capacity in people with COPD and identify which factors could affect the implementation of such a test in general practice. A mixed method feasibility study composed of a literature review and qualitative interviews was used. Quick functional tests for people with COPD were identified and evaluated through the COSMIN methodology. For the interviews, 64 general practices were included, and 50 staff members and 14 general practitioners (GPs) participated in the interviews. Responses were categorized and thematically analyzed. The 1 min sit-to-stand-test...

A < sup > 2 < /sup > BCD: a concise guide for asthma management
Lancet Respir Med. 2023 Jan 27:S2213-2600(22)00490-8. doi: 10.1016/S2213-2600(22)00490-8. Online ahead of print.ABSTRACTThe management of asthma has changed fundamentally during the past two decades. Precise assessment and phenotyping are now required to establish individually targeted treatment with disease-modifying anti-asthmatic drugs (DMAADs). Patients with asthma are often managed by primary care doctors or non-respiratory specialists in secondary care. However, the implementation of complex asthma guidelines in non-specialised care remains a challenge. There is a need for easy-to-understand, concise guides for gener...

Diagnostic differentiation between asthma and COPD in primary care using lung function testing
NPJ Prim Care Respir Med. 2022 Sep 5;32(1):32. doi: 10.1038/s41533-022-00298-4.ABSTRACTAsthma and COPD are defined as different disease entities, but in practice patients often show features of both diseases making it challenging for primary care clinicians to establish a correct diagnosis. We aimed to establish the added value of spirometry and more advanced lung function measurements to differentiate between asthma and COPD. A cross-sectional study in 10 Dutch general practices was performed. 532 subjects were extensively screened on respiratory symptoms and lung function. Two chest physicians assessed if asthma or COPD ...

A Delphi Survey of Canadian Respiratory Therapists' Practice Statements on Pediatric Mechanical Ventilation
CONCLUSIONS: This was the first study to survey RTs for their perspectives on the general practice of pediatric mechanical ventilation management in Canada, generally aligning with the ESPNIC guideline. These practice statements considered information from health organizations and institutes, supplemented with clinical remarks. Future studies are necessary to verify and understand these practices' effectiveness.PMID:35922069 | DOI:10.4187/respcare.09886

COVID-19 assessment in family practice-A clinical decision rule based on self-rated symptoms and contact history
NPJ Prim Care Respir Med. 2021 Nov 25;31(1):46. doi: 10.1038/s41533-021-00258-4.ABSTRACTThe study aimed to evaluate the diagnostic accuracy of contact history and clinical symptoms and to develop decision rules for ruling-in and ruling-out SARS-CoV-2 infection in family practice. We performed a prospective diagnostic study. Consecutive inclusion of patients coming for COVID-PCR testing to 19 general practices. Contact history and self-reported symptoms served as index test. PCR testing of nasopharyngeal swabs served as reference standard. Complete data were available from 1141 patients, 605 (53.0%) female, average age 42.2...
