What is the best approach in a patient with a failed aortic bioprosthetic valve: transcatheter aortic valve replacement or redo aortic valve replacement?
A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was whether transcatheter aortic valve-in-valve replacement (viv-TAVR) or redo aortic valve replacement (rAVR) is the best strategy in a patient with a degenerative bioprosthetic aortic valve. Altogether, 162 papers were found using the reported search, of which 12 represented the best evidence to answer the question. CF LVAD support is increasingly being used in the less ill patient phenotype and more patients are supported for destination therapy. Mean survival is now approaching 5 years, but adverse events, especially neurologic events, continue to have a detrimental impact on the success of CF LVAD support.

Failure to rescue contributes to center-level differences in mortality after lung transplantation
ConclusionsDifferences in rates of Failure to Rescue contribute significantly to per-center variability in mortality following LTx. FTR can serve as a quality metric to identify opportunities for improvement in management of perioperative adverse events.

The Specialized Donor Care Facility (SDCF) Model and Advances in Management of Thoracic Organ Donors.
CONCLUSIONS: Lack of high-quality prospective evidence results in conflicting practices across organ procurement organizations and best practices remain controversial. Future studies should focus on prospective, randomized investigations to evaluate donor management strategies. Controlled temperatures in cold preservation provides safe heart transplantation results
ConclusionUsing the SherpaPak system during heart transplantation is safe and not associated with significant recipient morbidity. None of the recipients experienced significant PGD and mortality.
