Stroke is the second most common cause of death worldwide and the fifth in the US, also representing the major cause of disability in older adults. We aim to determine the risk of acute ischemic stroke (AIS) in individuals with obesity, with past history of transient ischemic attack (TIA), compared with patients with a history of bariatric surgery (BaS).

Transcarotid artery revascularization (TCAR) using a flow-reversal neuroprotection system has gained popularity for the endovascular treatment of carotid artery atherosclerotic disease due to its lower risk of stroke or death compared with transfemoral carotid artery stenting. However, specific risk factors associated with stroke or death complications following TCAR have yet to be defined.

Primary Mechanism of Stroke Reduction in Transcarotid Artery Revascularization is Dynamic Flow Reversal
Recent studies have suggested that the low risk of stroke and death associated with TransCarotid Artery Revascularization (TCAR) is partially attributable to a robust dynamic flow reversal system as well as the avoidance of the atherosclerotic aortic arch during the stenting procedure. However, the benefit of flow reversal compared to distal embolic protection (DEP) in reducing stroke or death in transcarotid revascularization has not been studied.

Simultaneous Treatment of Common Carotid Lesions Increases the Risk of Stroke and Death after Carotid Artery Stenting
Tandem carotid artery lesions that involve simultaneous internal carotid artery (ICA) and common carotid artery (CCA) stenoses present a complex clinical problem. Some studies have shown that the addition of a retrograde proximal intervention to treat a CCA lesion during a carotid endarterectomy (CEA) increases the risk of stroke and death. However, the stroke and death risk associated with a totally endovascular approach to tandem lesions is unknown and is the subject of this study.

Surgical Thrombectomy After Large Vessel Ischaemic Stroke: A Case Report
Introduction: Endovascular mechanical thrombectomy (MT) changed acute ischaemic stroke (AIS) management. It is considered the standard of care for patients with proximal anterior circulation occlusion. MT improves functional independence, without increasing mortality; however, despite these advantages, up to 20% of patients treated with MT do not have a successful recanalisation, leading to a poor clinical outcome. Clinical Response to Procedural Stroke Following Carotid Endarterectomy: A  Delphi Consensus Study
No dedicated studies have been performed on the optimal management of patients with an acute stroke related to carotid intervention nor is there a solid recommendation given in the European Society for Vascular Surgery guideline. By implementation of an international expert Delphi panel, this study aimed to obtain expert consensus on the optimal management of in hospital stroke occurring during or following CEA and to provide a practical treatment decision tree.

Composite Dialysis, Paralysis, Stroke, or Mortality After Endovascular Aortic Interventions in the Society for Vascular Surgery Vascular Quality Initiative
Thoracoabdominal aortic aneurysm life-altering events, which include a combination of permanent dialysis, permanent spinal cord ischemia, stroke, and/or death, have devastating effects after complex endovascular repair. However, the occurrence of these life-altering events after endovascular abdominal aortic aneurysm repair (EVAR) and thoracic endovascular aortic repair (TEVAR) has not been studied. Therefore, we examined the effects of procedural and anatomic characteristics on a composite outcome of postoperative dialysis, stroke, paralysis, and/or mortality after any endovascular aortic repair.

Response to ‘Re. Risk of Stroke before Revascularisation in Patients with Symptomatic Carotid Stenosis: A Pooled Analysis of Randomised Controlled Trials’
We thank Prof. Abbott for her interest in our paper.1 We agree fully that the benefit of carotid endarterectomy (CEA) for symptomatic carotid stenosis in randomised trials depends on the patient characteristics that Prof. Abbott listed. However, our study focused on the risk of stroke before revascularisation among patients with symptomatic carotid stenosis in clinical trials. Of note, the risk of early recurrent stroke immediately after the first ischaemic event as reported in contemporary observational studies is higher than the corresponding risk following randomisation as measured in the trials we investigated.

Previous randomised trials have demonstrated the net benefit of surgery for significant symptomatic carotid stenosis but, with present day medical treatment, there is limited evidence on the risk of late ipsilateral ischaemic stroke (IS) and its main risk factors.

Carotid Endarterectomy After Intracranial Endovascular Thrombectomy for Acute Ischaemic Stroke in Patients with Carotid Artery Stenosis
Recent randomised controlled trials demonstrated the benefit of intracranial endovascular thrombectomy (EVT) in acute ischaemic stroke. There is no consensus, however, on how to treat concomitant extracranial carotid artery stenosis after EVT. The aim of this study was to evaluate the outcome in patients treated with carotid endarterectomy (CEA) after EVT, comparing complication rates among patients undergoing CEA for stroke without previous EVT.
