Evaluation of physician-modified endografts for the treatment of thoraco-abdominal and pararenal aortic pathologies at a single institution
CONCLUSIONS: When used by experienced teams under appropriate anatomical conditions, PMEGs are a safe and effective alternative to open surgery. However, further technical advancement and larger studies with long-term follow-up periods are warranted.PMID:34338495 | DOI:10.23736/S0021-9509.21.11877-4

Cardiac Operations after Transcatheter Aortic Valve Replacement
CONCLUSIONS: Cardiac operations after TAVR are increasing and interval between TAVR and operation decreasing. Most cardiac operations are complex, high-risk reoperations and isolated AVR rare. These findings should be considered when TAVR is selected for low-intermediate risk patients, particularly with multiple cardiac pathologies not addressed by TAVR.PMID:34800488 | DOI:10.1016/j.athoracsur.2021.10.022

Long ‐term outcomes of patients undergoing mechanical versus bioprosthetic aortic root replacement
ConclusionsThese findings suggest that mechanical valves for ARRs may confer a survival benefit over bioprosthetic valves. Surgeon bias was likely to account for this survival advantage.

Determining  optimal donor heart ischemic times in adult cardiac transplantation
ConclusionsThe donor heart IT threshold for survival determined from unbiased statistical modeling occurs at 3  h. With longer preservation times, transplantation with O donor hearts was associated with worse survival.

Outcomes of aortic root enlargement during isolated aortic valve replacement
ConclusionsARE during surgical AVR can be safely performed without an increase in complications with excellent early and intermediate-term survival.

Postoperative management in patients with complex aortic aneurysms
J Cardiovasc Surg (Torino). 2022 Jun 10. doi: 10.23736/S0021-9509.22.12359-1. Online ahead of print.ABSTRACTPatients with complex aortic aneurysms (CAA) are often high risk due to advanced age and widespread atherosclerosis affecting numerous vascular territories. Therefore, a thorough perioperative evaluation is needed prior to performing in any type of aortic repair, regardless of whether an endovascular or open surgical approach is selected. Because these operations are technically demanding and often result in end organ ischemia, it is not surprising that complex aortic repair carries significant risk of morbidity and ...

Type-A Aortic Dissection Repair in Patients With Prior Cardiac Surgery
CONCLUSIONS: Major adverse events after surgery for acute type A aortic dissection were more frequent in patients with previous cardiac surgery. Previous cardiac surgery itself was not an independent predictor for adverse events, although the small sample size precludes definite conclusions. Previous cardiac surgery should not deter from emergency surgery.PMID:35688205 | DOI:10.1016/j.athoracsur.2022.05.033
