A piece of the jigsaw of primary care: health professional perceptions of an integrated care model of hepatitis C management in the community.
CONCLUSION: The findings of this study suggest the need to address concerns of general practitioners regarding patient poaching. Key information to disseminate is the clinic's success in engaging with complex clients and contribution to more efficacious use of health service resources. These activities may require the advocacy of a key local opinion leader acting as 'knowledge broker'. PMID: 24892130 [PubMed - in process]

Monitoring the hepatitis C care cascade using administrative claims data.
CONCLUSIONS: With validated case-finding algorithms, insurance claims data can be used to describe and monitor portions of the HCV care cascade. Although nearly all enrollees with HCV were engaged in HCV care, only half received treatment, indicating that even commercially insured enrollees may find it challenging to access treatment. Evaluating HCV screening, linkage to care, and treatment across insurers.
CONCLUSIONS: Reducing steps in the HCV SLTC process increases the number of patients who learn their HCV status, receive appropriate care, and initiate treatment. PMID: 30130026 [PubMed - in process]

Hepatitis C care cascade among persons born 1945-1965: 3 medical centers.
CONCLUSIONS: This analysis showed that although linkage to care was largely successful in the target birth cohort, the largest gap in the HCV care cascade was seen in initiating treatment. Greater emphasis on linking patients to clinical evaluation and treatment is necessary in order to achieve the public health benefits promised by birth-cohort testing. A population approach to disease management: hepatitis C direct-acting antiviral use in a large health care system.
CONCLUSIONS: This clinically focused, comprehensive, population-based medication management approach affected real-time change in health services, practice, and outcomes evidenced by widespread and rapid DAA uptake, improved HCV RNA monitoring, attention to adherence, and more appropriate management of DAA-related anemia. Timely outcome sharing provided decision makers and clinicians evidence to support current HCV practices. PMID: 24856591 [PubMed - in process]
