Early Rehabilitation after Acute Stroke:The Golden Recovery Period
Acta Neurol Taiwan. 2022 Jan 18. Online ahead of print.ABSTRACTStroke is a leading cause of disability worldwide. Neuroplasticity, a condition wherein the brain's dynamic response to injury is heightened and rehabilitation might be effective, is observed shortly after acute stroke. However, although several trials have demonstrated that initiating treatment within 24 hours after stroke is potentially harmful, some have shown that early rehabilitation of patients is beneficial. Administration of constraint-induced movement therapy within two weeks after stroke appears to be beneficial for the upper extremities. In addition,...

Development of an educational program for staffs of emergency medical service to improve their awareness of stroke within 3 hours of symptom onset: a pilot study.
CONCLUSION: The educational program increased knowledge about stroke and improved the accuracy of triage by dedicated EMS providers. Further investigation is needed to determine the effectiveness of similar educational programs for non-dedicated EMS responders. PMID: 23479240 [PubMed - indexed for MEDLINE]

BNG-1 in the Recovery of Acute Ischemic Stroke: A phase 2, Double-blind, Placebo-controlled Randomized Trial.
CONCLUSIONS: BNG-1 trial was feasible, safe and well tolerated for patients with acute ischemic stroke based on the treatment of aspirin, though there was no statistically significant difference of efficacy between BNG-1 and placebo groups. A further large Phase 3 trial of BNG-1 is needed before recommending such treatments for general clinical use. PMID: 27333830 [PubMed - in process]

A Case of Fabry Disease Presenting with Young Stroke and Fever.
Authors: Wu LC, Chiang CT, Lee KF Abstract Although it is known that Fabry disease should be included in the differential diagnosis of young stroke and fever of unknown origin, it has not been reported in the literature of stroke with fever as the presentation of Fabry disease. This is relevant because stroke with fever may misguide the differential diagnosis. Here we present a young stroke patient with fever. DWI of brain MRI revealed acute multiple infarctions. Due to the fever, infectious and inflammatory origins such as infective endocarditis and autoimmune diseases were examined first. However, we could not id...

Acute Ischemic Stroke.
Authors: Rossi UG, Ierardi AM, Cariati M Abstract A 77-year-old woman with a history of hypertension developed acute onset of aphasia and right hemiplegia and hemisensory loss. She was urgently referred to emergency department. Cerebral multidetector computed tomographic angiography (MD-CTA) revealed an acute ischemic stroke due to the occlusion of the left middle cerebral artery (Figure 1). Since the symptoms started three hours previously, the patient was candidate for mechanical thrombectomy. The patient then performed a selective digital subtraction angiography (DSA) of the left internal carotid artery that con...

Clinical Updates on Antiplatelet Therapy for Secondary Prevention in Acute Ischemic Stroke
Acta Neurol Taiwan. 2023 Sep 30;32(3):138-144.ABSTRACTAntiplatelet therapy is the first-line management for noncardioembolic transient ischemic attack (TIA) and acute ischemic stroke (IS). Herein, we review the safety and efficacy of antiplatelet therapies in patients with IS and TIA, primarily focusing on the acute stage. We discuss current antiplatelet monotherapy and the factors influencing efficacy and continuation rate according to clinical trial data. Aspirin remains the most commonly used first-line antiplatelet agent for preventing noncardioembolic stroke recurrence, and clopidogrel, cilostazol, and ticagrelor are ...
