Interventional prevention of paradoxical embolism as the gold standard: End of discussion?
Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021 Jun 14. doi: 10.5507/bp.2021.036. Online ahead of print.ABSTRACTParadoxical embolism is one of the predominant causes of cryptogenic stroke and interventional secondary prevention, i.e., closure of the patent foramen ovale (PFO), is a much discussed issue. This review aims to provide a complex perspective on this topic, aggregates and comments on the available data and current guidelines. Several large trials were performed, some of which proved the superiority of PFO closure over pharmacotherapy while others have not. Studies detecting significant superiority of in...

Mechanical thrombectomy performed in thrombosed fusiform aneurysm after surgery for craniopharyngioma in adult: Case report
Biomed Pap Med Fac Univ Palacky Olomouc Czech Repub. 2021 Mar 31. doi: 10.5507/bp.2021.018. Online ahead of print.ABSTRACTINTRODUCTION: Fusiform dilatation of the internal carotid artery (ICA) is reported as a possible complication of craniopharyngioma resection in childhood. Here, the authors describe such a complication in an adult patient who presented with acute symptomatic thrombosis 7 months after surgery.MATERIALS AND METHODS: A 45-year-old woman presented with left hemispheric stroke due to a thrombotic supraclinoid occlusion of the terminal ICA (so called "T" occlusion). Successful revascularisation was achieved w...

Very late complications of oncotherapy in glioblastoma patients: A case series.
CONCLUSION: All stroke-like syndromes are diagnoses of exclusion. To avoid misinterpretation of imaging findings as glioblastoma recurrence and avert recall oncotherapy or redundant interventions, better understanding of delayed complications of brain tumor therapy is crucial. Cardiac papillary fibroelastomas: A 10-year single-center surgical experience and long-term echocardiographic follow-up study.
CONCLUSION: CPF can be safely and effectively treated surgically. TEE is superior to the TTE option in CPF post-operative recurrence detection. There is a clear need for a prospective study to determine criteria for embolization risk stratification and optimum management in patients with CPF. Subclinical atrial fibrillation - what is the risk of stroke?
Authors: Plasek J, Taborsky M Abstract Atrial fibrillation is the most common arrhythmia and as such, it has become a significant public health issue due to its impact on patient morbidity and mortality. The prevalence of atrial fibrillation (AF) almost doubled in the last decade, being currently 2% in unselected patient populations. Its occurrence varies with age (present in almost 20% of octogenarians) and concomitant diseases. The most prevalent concomitant diseases are hypertension, diabetes, heart failure, renal failure, and cognitive decline. Cognitive decline or stroke may be actually the first manifestation...

Functional differences in upper limb movement after early and chronic stroke based on kinematic motion indicators.
CONCLUSION: The variables used in the study showed applicability in assessing kinematic parameters in both the early and chronic period after stroke. PMID: 30338767 [PubMed - as supplied by publisher]

Midterm outcomes of mini-invasive surgical and hybrid ablation of atrial fibrillation.
CONCLUSIONS: . Combination of mini-invasive surgical and endocardial treatment (two-stage hybrid procedure) is a safe and effective method for the treatment of isolated (lone) AF. This procedure provided good midterm outcomes. PMID: 30214077 [PubMed - as supplied by publisher]

Safety of early discharge in low risk patients after acute ST-segment elevation myocardial infarction, treated with primary percutaneous coronary intervention. Open label, randomized trial.
CONCLUSION: This study confirms that early discharge within 48-72 h in selected low risk patients after STEMI treated with successful primary PCI is feasible and safe, with outcomes comparable to the later discharge. This strategy applies to more than a quarter of all STEMI patients. PMID: 30181666 [PubMed - as supplied by publisher]

Early manifestation of spasticity after first stroke in the territory of the internal carotid artery: A prospective multicenter study.
CONCLUSION: Spasticity was noted in 44.9% patients with neurological deficit due to first-ever stroke in the carotid territory in the first 10 days after stroke onset. Severe spasticity was rare. PMID: 29795545 [PubMed - as supplied by publisher]

Vulnerable atherosclerotic plaque - review of current concepts and advanced imaging.
Authors: Spacek M, Zemanek D, Hutyra M, Sluka M, Taborsky M Abstract Atherosclerosis is the most common cause of both carotid and coronary steno-occlusive disease. Rupture of an atherosclerotic plaque may lead to the formation of an overlying thrombosis resulting in complete arterial occlusion or downstream embolism. Clinically, this may manifest as a stroke or acute myocardial infarction, the overall leading causes of mortality and disability in developed countries. In this article, we summarize current concepts of the development of vulnerable plaque and provide an overview of commonly used imaging methods that m...

Electrical stimulation of hyoid muscles in post-stroke dysphagia.
CONCLUSION: Electrical stimulation of suprahyoid muscles significantly reduced the duration of the oral and pharyngeal phases: in the post-stroke patients with dysphagia used in this study. The result is improved swallowing. This is a recently recommended approach and your study confirms its efficacy. PMID: 29097820 [PubMed - as supplied by publisher]
