Oral Anticoagulation in Chronic Kidney Disease and Atrial Fibrillation.
CONCLUSION: The cardiological societies' recommendation that patients with atrial fibrillation should be given oral anticoagulant drugs applies to the majority of such patients who also have chronic kidney disease. PMID: 29789105 [PubMed - in process]

The Screening, Diagnosis, Treatment, and Follow-Up of Breast Cancer.
CONCLUSION: Consistent implementation of the recommendations in the newly updated guideline can help lessen morbidity and mortality from breast cancer. The actual extent to which breast cancer guidelines are implemented should be a topic of future research. PMID: 29807560 [PubMed - in process]

Alcohol Biomarkers in Clinical and Forensic Contexts.
CONCLUSION: The available alcohol biomarkers vary in sensitivity and specificity with respect to the time period over which they indicate alcohol use and the minimum extent of alcohol use that they can detect. The appropriate marker or combination of markers should be chosen in each case according to the particular question that is to be answered by laboratory analysis. PMID: 29807559 [PubMed - in process]

Arrhythmia-Induced Cardiomyopathy.
CONCLUSION: An AIC is potentially reversible. The timely recognition of this condition and the appropriate treatment of theunderlying arrhythmia can substantially improve patient outcomes. PMID: 29875055 [PubMed - in process]

The Differential Diagnosis and Treatment of Thrombotic Microangiopathies.
CONCLUSION: Rapid differential diagnosis is needed in order to determine the specifictype of thrombotic microangiopathy that is present, because only patients with TTPand only a very small percentage of those with atypical hemolytic-uremic syndrome(aHUS) can benefit from plasmapheresis. The establishment of a nationwideregistry in Germany with an attached biobank might help reveal yet unknowngenetic predispositions. PMID: 29875054 [PubMed - in process]

The Treatment of Gliomas in Adulthood.
CONCLUSION: The consistent application of the existing multimodal treatment options for glioma has led in recent years to improved survival. Areas of important current and future scientific activity include immunotherapy and targeted and combined chemotherapy, as well as altered neurocognition, modern approaches to palliative care, and complementary therapies. PMID: 29914619 [PubMed - in process]

Treatment Options for Carbapenem- Resistant Gram-Negative Infections.
CONCLUSION: The treatment of severe infection with carbapenem-resistant pathogens should be individualized and carried out in an interdisciplinary framework, in consideration of antibiotic pharmacokinetics and pharmacodynamics in each case. The treat- ment options are based on evidence from in vitro studies, retrospective studies, and case series, which must be interpreted with caution. Randomized clinical trials are needed to test each of the various combined approaches. PMID: 29914612 [PubMed - in process]

Strategies for Treating Scoliosis in Early Childhood.
CONCLUSION: Because of the low prevalence and heterogeneous etiology of early childhood scoliosis, the literature to date contains no randomized controlled therapeutic trials concerning this small group of high-risk patients. For the treatment to succeed, it is essential for specialists from all of the involved medical disciplines to work closely together. Conservative measures such as physiotherapy, casts, and corsets can delay the (frequent) need for surgery or even make surgery unneces - sary, particularly in the idiopathic types of early childhood scoliosis. The new non-fusion techniques enable continued growth of the ...

Chronic Headache Due to Overuse of Analgesics and Anti-Migraine Agents.
CONCLUSION: Patients who frequently take acute medication to treat headache episodes must be identified early in order to avoid headache chronification and medication overuse headache. The suggested treatment algorithm is still in need of validation by randomized trials. PMID: 29932046 [PubMed - in process]

Exercise Testing in Sports Medicine.
CONCLUSION: Ergometric stress tests provide important data in clinical and preventive medicine. The findings are often difficult to interpret because of the wide range of normal findings, the use of different stress-inducing protocols, and the lack of gen- erally accepted reference values. The establishment of a nationwide fitness and health registry for ergometric data would be very helpful for the individualized inter- pretation of test findings and for the monitoring of exercise training and therapy. Helicobacter Pylori Infection.
CONCLUSION: The new, clinically relevant developments that are presented and commented upon in this review now enable evidence-based management of H. pylori infection. PMID: 29999489 [PubMed - in process]

The Diagnosis and Treatment of Primary CNS Lymphoma.
CONCLUSION: Current recommendations for the treatment of primary CNS lymphomaare based on only a small number of prospective clinical trials. Patients with this disease should be treated by interdisciplinary teams in experienced centers, andpreferably as part of a controlled trial. PMID: 29999484 [PubMed - in process]

The Diagnosis and Treatment of Pacemaker-Associated Infection.
CONCLUSION: Untreated infection carries a high mortality. Evaluation and treatment according to a standardized clinical algorithm facilitate correct and timely diagnosis and the choice of an appropriate therapeutic strategy. PMID: 30017027 [PubMed - in process]

Video Versus Direct Laryngoscopy for Inpatient Emergency Intubation in Adults.
CONCLUSION: The routine use of VL for airway management in emergency medicine might improve patient safety, as VL is associated with a lower number of intubation attempts and with a lower frequency of esophageal intubation. Further randomized controlled trials are needed before any definitive conclusions can be drawn about the advantages of video laryngoscopy. PMID: 30017026 [PubMed - in process]

Oxygen Treatment in Intensive Care and Emergency Medicine.
CONCLUSION: Hypoxemia should certainly be avoided, but the fact that the liberal administration of oxygen to patients in intensive care units and emergency rooms tends to increase morbidity and mortality implies the advisability of a conservative, normoxic oxygenation strategy.
