The Endoscopic Treatment of Iatrogenic Gastrointestinal Perforation.
CONCLUSION: The endoscopic treatment of iatrogenic perforations is safe and reliable. Success depends on early detection, adequate endoscopic closure with properly mastered technique, and the early initiation of concomitant antibiotic treatment, which must be continued for a full course. Most patients who are treated in this way do not need emergency surgery. PMID: 26976712 [PubMed - in process]

Evidence-Based Hernia Treatment in Adults.
CONCLUSION: The choice of procedure has been addressed by international guidelines based on high-level evidence. Surgeons should deviate from their recommendations only in exceptional cases and for special reasons. Guideline conformity implies that hernia surgeons must master both open and endoscopic/laparoscopic techniques. PMID: 26987468 [PubMed - in process]

Acute Lumbar Back Pain.
CONCLUSION: After dangerous conditions have been ruled out, low back pain can be pragmatically classified as either nonspecific or specific. More research is needed so that the diagnostic assessment and individualized treatment of acute lower back pain can be further refined. PMID: 27120496 [PubMed - as supplied by publisher]

Dependence on Prescription Opioids.
CONCLUSION: Physicians who know the indications and risks of opioid therapy and the typical behavior of drug-dependent patients will be better able to identify patients at risk and to prevent dependence. Studies on the prevalence of opioid abuse and dependence in German patients with chronic pain can help provide better estimates of the current extent and implications of this problem in Germany. Mental Health Problems in a School Setting in Children and Adolescents.
CONCLUSION: Physicians, in collaboration with school social workers and psychologists, should help teachers recognize and contend with mental health problems among the children and adolescents whom they teach, to enable the timely detection of stress factors at school and the initiation of the necessary measures and aids. In particular, the school-entrance examination and screening for risk factors at school can make a positive contribution. Evidence-based preventive programs should be implemented in schools, and beneficial changes of the school environment should be a further goal. PMID: 27118666 [PubMed - in process]

Radiotherapy and Hormone Treatment in Prostate Cancer.
CONCLUSION: Androgen deprivation combined with external beam radiotherapy is a curative standard option for patients with prostate cancer who are at high risk of recurrence. The modern radiotherapeutic techniques that are now available, such as intensity-modulated radiotherapy, enable a further improvement of the risk/benefit ratio. PMID: 27146591 [PubMed - in process]

Infection Risk in Sterile Operative Procedures.
CONCLUSION: A multimodal approach with the participation of specialists from various disciplines can further reduce the rate of postoperative infection. A reduction to zero is not realistic and is not supported by available evidence. PMID: 27159141 [PubMed - as supplied by publisher]

The Efficacy, Safety and Applications of Medical Hypnosis.
CONCLUSION: Medical hypnosis is a safe and effective complementary technique for use in medical procedures and in the treatment of irritable bowel syndrome. Waking suggestions can be a component of effective doctor-patient communication in routine clinical situations. PMID: 27173407 [PubMed - in process]

Clonal Hematopoiesis of Indeterminate Potential.
CONCLUSION: CHIP must be included in the differential diagnosis of peripheral blood cytopenia. This new entity can help us understand the clinical significance of clonal hematopoiesis. PMID: 27215596 [PubMed - in process]

The Pharmacological Treatment of Chronic Obstructive Pulmonary Disease.
CONCLUSION: ICS monotherapy has no clinically useful effect on pulmonary function in COPD. The main form of drug treatment for COPD is with broncho - dilators, either alone or in combination with ICS. ICS can be given to patients with grade III or IV COPD to make exacerbations less frequent. Patients with an asthma-COPD overlap syndrome (ACOS) can benefit from ICS treatment. Lichen Sclerosus-Presentation, Diagnosis and Management.
CONCLUSION: Anogenital itching and clinical features such as erythema, white skin changes (such as hyperkeratosis and sclerosis), and fissures should arouse suspicion of lichen sclerosus. The diagnosis should be confirmed with a skin biopsy, and early, thorough treatment should be initiated. In this way, a mutilating disease course can be averted, and the risk of cancer can be lessened. PMID: 27232363 [PubMed - as supplied by publisher]

Fine Needle Aspiration in the Investigation of Thyroid Nodules.
CONCLUSION: Fine needle aspiration biopsy of the thyroid gland in experienced hands is an easily performed diagnostic procedure with very little associated risk. It should be performed on ultrasonographically suspect nodules for treatment stratification and before any operation for an unclear nodular change in the thyroid gland. Improving Treatment Adherence in Heart Failure.
CONCLUSION: Many effective interventions are available that can lead to sustained improvement in patient adherence and in clinical endpoints. Longterm success depends on patients' assuming responsibility for their own health and can be achieved with the aid of coordinated measures such as patient education and regular follow-up contacts. CONCLUSION: In high-risk patients with an acute coronary syndrome, combination therapy with ezetimibe and a statin lowered the risk of cardiovascular events in comparison to statin monotherapy. The risk of dying or suffering an adverse drug effect was similar in the two treatment groups. PMID: 27412989 [PubMed - in process]
