Aphasia with No Apparent Paralysis in Progressive Stroke of the Anterior Choroidal Artery
Intern Med. 2022 Aug 30. doi: 10.2169/internalmedicine.0009-22. Online ahead of print.ABSTRACTSome anterior choroidal artery (AChA) infarctions in the posterior limbs of the internal capsule (plIC) have been reported to cause aphasia, typically with apparent paralysis. We herein report an 84-year-old woman with AChA infarction. Although her dysarthria remained mild with no apparent paralysis, we overlooked progression to branch atheromatous disease-related infarct with exacerbation of her anomia, which delayed the initiation of more intense therapy. Even in AChA infarction, especially when the lesion is located mainly in t...

Features of Cerebral Infarction Due to Left Ventricular Thrombus
Conclusion LV thrombus was seen in 0.9% of patients with CES. Many of the patients with LV thrombus had major vessel occlusion.PMID:36047094 | DOI:10.2169/internalmedicine.8015-21

Moyamoya Syndrome Presenting with Ischemic Stroke Caused by Rapid Worsening of Vessel Stenosis Associated with Triple-seropositive Antiphospholipid Syndrome: A Case Report
Intern Med. 2022 Aug 10. doi: 10.2169/internalmedicine.9316-22. Online ahead of print.ABSTRACTSeveral case reports of patients with both moyamoya syndrome (MMS) and antiphospholipid syndrome (APS) have been published. However, the relationship between MMS and APS has not been clarified. We herein report a patient with MMS who had an ischemic stroke with rapid worsening of stenosis of the middle cerebral artery associated with APS. The patient was triple-positive for antiphospholipid antibodies. Patients with MMS complicated by APS should be closely followed up with vascular imaging.PMID:35945020 | DOI:10.2169/internalmedicine.9316-22

Ischemic Stroke Due to Heparin-induced Thrombocytopenia During Severe COVID-19 Infection
Intern Med. 2022 Jul 5. doi: 10.2169/internalmedicine.9531-22. Online ahead of print.ABSTRACTA 53-year-old woman with severe coronavirus disease 2019 (COVID-19) pneumonia was admitted and treated with intravenous unfractionated heparin for thromboprophylaxis under general anesthesia with mechanical ventilation. She developed right hemiparesis after hospitalization due to a large hemorrhagic infarction. Her platelet count decreased from 243,000/μL at administration to 121,000/μL. Anti-platelet factor 4-heparin antibody testing was positive according to a latex immunoturbidimetric assay. She was therefore diagnosed with he...
