The effect of prehospital P2Y12 receptor inhibition in primary percutaneous coronary intervention for ST-segment elevation myocardial infarction: the ATLANTIC-Elderly analysis.
CONCLUSIONS: Elderly patients who represented one sixth of the patients randomized in the ATLANTIC trial, had less successful mechanical reperfusion and a six-fold increase in mortality at 30 days, likely due to comorbidities and possible undertreatment. The impact of chronic kidney disease on outcomes following percutaneous coronary interventions versus coronary artery bypass grafting in patients with complex coronary artery disease: 5-year follow-up of the SYNTAX trial.
CONCLUSIONS: During a 5-year follow up, adverse event rates were comparable between PCI and CABG patients with moderate CKD but significantly higher compared to the patients with impaired or normal kidney function. The negative impact of the CKD on long-term outcome following PCI appears to be stronger when compared to CABG, especially in the CKD patients with diabetes and extensive coronary disease. Clopidogrel or ticagrelor in acute coronary syndrome patients treated with newer-generation drug-eluting stents: CHANGE DAPT.
CONCLUSIONS: In this observational study, the guideline-recommended ticagrelor-based primary DAPT regimen was associated with an increased event risk in consecutive ACS patients treated with newer-generation DES. PMID: 29151439 [PubMed - in process]

Clinical events beyond one year after an acute coronary syndrome: insights from the RECLOSE 2-ACS study.
CONCLUSIONS: Patients at risk of adverse events beyond 12 months after an ACS may be identified by simple clinical and angiographic characteristics such as age, diabetes, chronic kidney disease, prior MI and multivessel CAD. Hierarchical testing of composite endpoints: applying the win ratio to percutaneous coronary intervention versus coronary artery bypass grafting in the SYNTAX trial.
CONCLUSIONS: This re-analysis of the SYNTAX trial using the win ratio shows that the most important benefit of CABG treatment is the reduction of hard clinical endpoints like mortality and MI. Future trials using this approach can expect to maintain similar statistical power with smaller sample sizes, and thereby reduce the cost of a trial. Unfractionated heparin versus bivalirudin in patients undergoing primary percutaneous coronary intervention: a SWEDEHEART study.

Design and rationale for a randomised comparison of everolimus-eluting stents and coronary artery bypass graft surgery in selected patients with left main coronary artery disease: the EXCEL trial.

Five-year outcomes of staged percutaneous coronary intervention in the SYNTAX study.
CONCLUSIONS: There is a higher incidence of MACCE in patients undergoing staged compared to single-session PCI for 3-vessel and/or left main disease over the first five years of follow-up. However, these patients had more comorbidities and more diffuse disease. PMID: 25912390 [PubMed - in process]
