A randomized controlled trial to compare the safety and efficacy of sirolimus-eluting biodegradable polymer ultra-thin stent (SUPRAFLEX Cruz) and everolimus-eluting biodegradable polymer stent (SYNERGY) in treatment for three-vessel coronary artery disease: design of the Multivessel TALENT trial.
CONCLUSIONS: The Multivessel TALENT trial will be evaluating a novel treatment strategy for complex coronary artery disease with state-of-the-art PCI based on angiography-derived QFR with novel ultra-thin SUPRAFLEX Cruz stents, compared with the SYNERGY stents. Long-Term Outcomes of In-Stent Restenosis Percutaneous Coronary Intervention among Medicare Beneficiaries.
CONCLUSIONS: ISR PCI was common and associated with significantly higher risk of recurrent long-term major ischemic events compared to patients undergoing de novo lesion PCI. There remains a need for new strategies to minimize ISR. PMID: 32863243 [PubMed - as supplied by publisher]

A Prospective, Multicenter, Randomized, Open-label Trial to Compare Efficacy and Safety of Clopidogrel vs. Ticagrelor in Stabilized Patients with Acute Myocardial Infarction after Percutan eous Coronary Intervention: rationale and design of the TALOS-AMI trial.
CONCLUSIONS: The TALOS-AMI trial is the first large-scale, multicenter, randomized study exploring the efficacy and safety of the de-escalating antiplatelet therapy that switches ticagrelor to clopidogrel in stabilized AMI patients undergoing PCI. PMID: 32718912 [PubMed - as supplied by publisher]

Cardiovascular mortality and morbidity in patients undergoing percutaneous coronary intervention after out-of-hospital cardiac arrest: a systematic review and meta-analysis.
CONCLUSIONS: Almost one third of OHCA patients undergoing PCI die and more than one third of the fatalities are attributable to cardiovascular causes. The burden of ischemic and bleeding complications was consistently higher and the success rates of PCI lower among OHCA as compared to non-OHCA patients. PMID: 32624462 [PubMed - as supplied by publisher]

Fractional Flow Reserve-Guided Multivessel Angioplasty in Myocardial Infarction: 3-year follow-up with cost benefit analysis of the Compare-Acute trial.
CONCLUSIONS: In patients with STEMI and MVD, FFR-guided complete revascularization is more beneficial in terms of outcome and health-care costs compared to IRA-only revascularization at 36 months. PMID: 32250250 [PubMed - as supplied by publisher]

Clinical outcomes of PCI with rotational atherectomy: the European multicentre Euro4C registry.
CONCLUSIONS: Despite the high level of complexity of the studied population, RA turned out to be an effective procedure with a low rate of in-hospital complications and demonstrated good immediate and mid-term results. PMID: 32250249 [PubMed - as supplied by publisher]

Procedural Characteristics and Clinical Outcomes in Patients Undergoing Percutaneous Coronary Intervention for Left Main Trifurcation Disease: The EXCEL trial.
CONCLUSIONS: Despite the greater inherent complexity, procedural and long-term clinical outcomes following PCI of distal LM trifurcations with everolimus-eluting stents in a modest-sized cohort from the EXCEL trial were similar compared with treatment of distal LM bifurcation disease without trifurcations. These findings support PCI as a treatment strategy for selected patients with distal LM trifurcation disease. PMID: 31793882 [PubMed - as supplied by publisher]

Influence of Final Kissing Balloon Inflation on Long-term Outcomes After PCI of Distal Left Main Bifurcation Lesions: Analysis From the EXCEL Trial.
CONCLUSIONS: In the EXCEL trial, the performance of FKBI after PCI of distal LM bifurcation lesions was not associated with improved 4-year clinical outcomes regardless of whether 1 stent or ≥2 stents were implanted. PMID: 31763982 [PubMed - as supplied by publisher]

Treatment with a dedicated bifurcation sirolimus-eluting cobalt-chromium stent for distal left main coronary artery disease: rationale and design of the POLBOS LM study.
CONCLUSION: The POLBOS LM study will indicate the efficacy of BiOSS LIM C stent with contemporary PCI for distal left main bifurcation lesions in comparison with the XIENCE stent from the recent EXCEL trial, as a performance index. PMID: 31543500 [PubMed - as supplied by publisher]

Society of Thoracic Surgeons Risk Scores Performance in Patients with Left Main Coronary Artery Disease Undergoing Revascularization in the EXCEL trial.
CONCLUSIONS: In selected patients with LMCAD from the EXCEL trial, STS risk models showed good predictive performance for CABG yet lacked predictive performance for PCI for perioperative mortality and renal failure. The STS stroke risk model was surprisingly more discriminating in PCI compared to CABG EXCEL patients. Improved and procedure-specific risk-prediction instruments are needed to accurately estimate adverse events after LMCAD revascularization by CABG and PCI. Comparative Effectiveness Analysis of Percutaneous Coronary Intervention vs. Coronary Artery Bypass Grafting in Patients with Chronic Kidney Disease and Unprotected Left Main Coronary Artery Disease :Insights From a Large-Sized All-Comers Registry.
CONCLUSIONS: The degree of renal insufficiency was proportionately associated with unfavorable outcomes in patients with LMCAD. In group 3, PCI was associated with a higher risk of MACCE compared with CABG. Also, the effect of PCI vs. CABG on MACCE was consistent, with PCI being associated less bleeding and CABG being associated with less repeat revascularization. Does an Occluded RCA Affect Prognosis in Patients Undergoing PCI or CABG for Left Main Coronary Artery Disease? Analysis From the EXCEL Trial.
CONCLUSIONS: In the EXCEL trial, the presence of an occluded RCA at baseline did not confer a worse 3-year prognosis in patients undergoing revascularization for LMCAD and did not affect the relative outcomes of PCI versus CABG in this high-risk patient cohort. PMID: 31186220 [PubMed - as supplied by publisher]

Standard versus ultrasound-guided radial and femoral access in coronary angiography and intervention (SURF): a randomised controlled trial.
CONCLUSIONS: Transradial access significantly reduced the composite outcome compared to transfemoral access. Ultrasound guidance did not reduce the primary outcome compared to the standard technique, but significantly improved the efficiency and overall success rate of arterial access. PMID: 31113763 [PubMed - as supplied by publisher]

Diabetes and Outcomes following Guided De-Escalation of Antiplatelet Treatment in Acute Coronary Syndrome Patients Undergoing Percutaneous Coronary Intervention: A Prespecified Analysis from the Randomized TROPICAL-ACS Trial.
CONCLUSIONS: Although diabetic status did not significantly interfere with treatment effects of guided DAPT de-escalation, our results suggest that this approach might be safe and effective in non-diabetic patients, whilst further investigation is definitely warranted in diabetic patients. PMID: 31012853 [PubMed - as supplied by publisher]

Patient-oriented composite endpoints and net adverse clinical events with ticagrelor monotherapy following percutaneous coronary intervention: Insights from the randomized GLOBAL LEADERS trial.
CONCLUSIONS: The experimental treatment strategy of one-month DAPT followed by 23 months of ticagrelor alone did not result in a significant reduction in the rates of site-reported POCE or NACE, when compared to the reference treatment. PMID: 30888959 [PubMed - as supplied by publisher]
