Females are Less Likely to Receive Best Medical Therapy for Stroke Prevention Before and After Carotid Revascularization than Males
Current professional guidelines recommend best medical therapy (BMT) with statin agents and antiplatelet therapy for primary and secondary stroke prevention in patients with carotid artery stenosis. We aimed to assess the association of patient sex with preoperative BMT in patients undergoing carotid revascularization.

The imperative need to identify stroke risk stratification models for patients with asymptomatic carotid artery stenosis
The optimal management of patients with asymptomatic carotid stenosis (ACS) is controversial. Invited commentary
Although carotid endarterectomy and stenting for symptomatic significant carotid artery stenosis are widely accepted for stroke prevention, their use for asymptomatic stenosis is more controversial. With the advances in medical therapy, particularly the use of statins, and with an annual stroke risk of 1% to 2% for asymptomatic disease, justifying these interventions in these patients is more difficult. What is more disturbing is that ∼90% of all carotid endarterectomy and stenting procedures in the United States are performed on asymptomatic patients and that many were felt to be unnecessary, resulting in a cost of>$2 billion annually.

FT19. Combination of Preoperative CT and Carotid Plaque Gene Expression Profiling Demonstrates an Association Between Calcification and Plaque-Stabilizing Processes
Unstable carotid stenosis is a major cause of stroke, but we lack methods to identify patients and lesions at risk. We recently described enrichment of molecular pathways associated with vascular calcification in carotid plaques from asymptomatic patients and from patients on statins. Here, we tested the hypothesis that calcification is a stabilizing feature in carotid atherosclerosis and examined whether calcification estimated by computed tomography (CT) can reflect plaque phenotype.

Adherence to lipid management guidelines is associated with lower mortality and major adverse limb events in patients undergoing revascularization for chronic limb-threatening ischemia
The 2013 American College of Cardiology/American Heart Association lipid management guidelines recommend high-intensity statins for all patients ≤75 years old with chronic limb-threatening ischemia (CLTI) and moderate-intensity statins for CLTI patients>75  years old without contraindications or on dialysis, but these recommendations are based primarily on coronary and stroke data. We aimed to validate these guidelines in patients with CLTI and to assess current adherence to these recommendations.

Increased Stroke Rate in Medically Managed Carotid Stenosis Patients
Since the time of the Asymptomatic Carotid Artery Stenosis (ACAS) trial, medical management of carotid stenosis has improved significantly. With the widespread use of antiplatelet agents and statins, it is no longer clear that patients with 60% internal carotid artery (ICA) stenosis will benefit from carotid endarterectomy (CEA). The Vascular Quality Initiative registry has shown that there are wide practice variations across the country regarding the degree of stenosis that prompts consideration for surgical intervention.

Effects of Statins on Early and Late Clinical Outcomes of Carotid Endarterectomy and the Rate of Post-Carotid Endarterectomy Restenosis
Statins have no effect on post-carotid endarterectomy (CEA) restenosis but are associated with decreased death rates in patients with diabetes trending to lower both death and stroke rates in diabetic patients with hypercholesterolemia.

Long-term Mortality in Patients with Asymptomatic Carotid Stenosis: Implications for Statin Therapy
