Cerebral Infarction in Both Carotid and Vertebrobasilar Territories Associated with a Persistent Primitive Hypoglossal Artery with Severe Dilated Cardiomyopathy
A persistent primitive hypoglossal artery (PPHA) is a relatively rare vascular anomaly of a persistent carotid–basilar anastomosis. A 76-year-old man with severe dilated cardiomyopathy suddenly lost consciousness. A magnetic resonance imaging scan of his brain revealed extensive infarction in the carotid and vertebrobasilar territories. Cerebral Blood Flow and Oxygen Metabolism Measurements Using Positron Emission Tomography on the First Day after Carotid Artery Stenting
Conclusions: CAS increases cerebral perfusion and improves hemodynamic compromise in patients with symptomatic ICA stenosis. Although we could not clarify the usefulness of PET before and on the first day after CAS in predicting HPS, a high preoperative OEF is related to postoperative marked CBF increase and might be used as a predictor of HPS. Patients with greater hemodynamic compromise with a high preoperative OEF should be managed carefully to prevent HPS, but they have a greater chance of CBF increase after CAS.

Magnetic Resonance Imaging Versus Computed Tomography for Identification and Quantification of Intraventricular Hemorrhage
Conclusions: MRI identifies small volumes of IVH in cases not detected by CT and yields higher estimates of intraventricular blood volume. These data indicate that consideration of technical differences is needed when comparing images from the 2 modalities in the evaluation for IVH.

Intracranial Vascular Calcification is Protective from Vasospasm after Aneurysmal Subarachnoid Hemorrhage
The objective of this study was to assess the association between intracranial calcification, a marker for atherosclerosis, and vasospasm. Charts and nonenhanced computed tomography scans of patients with subarachnoid hemorrhage were retrospectively reviewed. Transcranial Doppler studies were used to categorize vasospasm using mean flow velocity: mild vasospasm 120-199 cm/second and severe ≥200 cm/second.
