Nonalcoholic Fatty Liver Disease in Patients with Acute Ischemic Stroke is associated with more severe stroke and worse outcome.
There is a paucity of data regarding the association between non alcoholic fatty liver disease (NAFLD) and acute ischemic stroke. Stroke is largely preventable, so that knowledge of risk factors is essential to achieve reductions in the stroke rate and resulting disease burden.

Admission LDL-cholesterol, statin pretreatment and early outcomes in acute ischemic stroke
Elevated LDL-cholesterol (LDL-C) levels increase the risk of stroke and vascular events. Initiation and maintenance of lipid-lowering therapy after acute ischemic stroke (AIS) appears to be guided by LDL-C levels.1 The EUROASPIRE III stroke-specific module shows that secondary prevention and risk factor control in patients after ischemic stroke need to be improved since about half of patients are not achieving target lipid levels defined in European guidelines.2 However, lower levels of LDL-C are paradoxically associated with increased mortality in ST elevation myocardial infarction patients.

Primary Prevention of Stroke is Strongly Correlated with Low-Density-Lipoprotein Cholesterol Reduction: A Meta-Regression Analysis
Statins have become the cornerstone of therapy for both primary and secondary prevention of cardiovascular (CV) and cerebrovascular diseases (CVD). Numerous studies and meta-analyses have documented these benefits independent of low-density-lipoprotein cholesterol (LDL-C) lowering. There are discrepant reports regarding the dependence of the stroke benefits on the baseline LDL-C levels. The relevance of LDL-C reduction from baseline in the era of pleiotropic benefits needs further exploration, especially, while comparing the primary stroke cohort with the secondary stroke prevention cohort.

Low LDL-C and risk of hemorrhagic stroke in Chinese adults
It is well-established that higher LDL and total cholesterol are associated with increased risk of atherosclerotic cardiovascular disease (ASCVD), that is, coronary heart disease (CHD) and ischemic stroke events, and that cholesterol-lowering by statins and other therapies reduce ASCVD risk. However, the picture is less clear for the 10% to 20% of strokes that are hemorrhagic rather than ischemic. A meta-analysis of observational studies shows that lower levels of LDL-C and total cholesterol are associated with higher risk of hemorrhagic strokes (HS), and with intracranial hemorrhage (ICH), a subset of HS.
