Major depressive disorder in chronic heart failure patients: Does silent cerebral infarction cause major depressive disorder in this patient population?
CONCLUSION: CHF patients have an increased prevalence of SCI and MDD. Patients with SCI have a higher prevalence of MDD compared to patients without SCI in CHF. PMID: 26363742 [PubMed - in process]

Resolution of left ventricular thrombus with apixaban in a patient with hypertrophic cardiomyopathy.
Abstract Anticoagulation with warfarin is the main treatment of intracardiac thrombus. While novel oral anticoagulants (NOACs) have been approved by the US Food and Drug Administration (FDA) for stroke prevention in patients with nonvalvular atrial fibrillation (AF), they are not indicated for resolution of intracardiac thrombus. The case of a 60-year-old woman with left ventricular (LV) thrombus secondary to hypertrophic cardiomyopathy and AF is described in the present report. Indications for oral anticoagulation were AF and LV thrombus. Thrombus was dissolved after 1 month of apixaban treatment. To the best of ...

Turkish registry for diagnosis and treatment of acute heart failure: TAKTIK study.
CONCLUSION: Compared to previous data collected around the world, AHF patients in Turkey were younger, had more frequently valvular heart disease as the underlying cause, and were more noncompliant with medical treatment, but overall mortality was lower. Drugs shown to reduce mortality, and which also form the basis of guideline-directed medical therapy, are still used inadequately. PMID: 28045409 [PubMed - in process]

All diuretics used in the treatment of hypertension are not the same.
Abstract Diuretics have been used for years to treat hypertension as both a monotherapy and in combination. Hydrochlorothiazide, indapamide, and chlorthalidone have frequently been considered in the same category as thiazide diuretics, but there is no evidence that their activities are similar. Studies have shown that chlorthalidone and indapamide reduce cardiovascular morbidity and mortality; however, there is no study indicating that hydrochlorothiazide has beneficial effects on cardiovascular outcomes such as myocardial infarction, renal failure, stroke, or death. Hydrochlorothiazide has less effect on blood pr...

Esophageal ulcers secondary to cryoenergy after pulmonary vein ablation using the second-generation balloon.
Abstract Pulmonary vein isolation is an effective treatment for patients with symptomatic paroxysmal atrial fibrillation. Cryoablation balloon therapy has been developed as an alternative. Cryoablation complications have primarily been related to phrenic nerve palsy, vascular complications, stroke, and others of lesser incidence. Esophageal lesions are rare and they are not yet completely understood. PMID: 29664429 [PubMed - in process]

Left atrial appendage closure using Amulet device in a patient with prior percutaneous atrial septal defect closure.
Abstract Percutaneous left atrial appendage (LAA) occlusion is an alternative therapeutic option for stroke prevention in patients with atrial fibrillation and contraindications for oral anticoagulation. There are few data available regarding the technical success of percutaneous LAA closure in patients with the previous implantation of an atrial septal defect (ASD) or a patent foramen ovale closure device. This is the description of a case of a successful LAA closure performed with an Amplatzer Amulet occluder device (St. Jude Medical, Inc., St. Paul, MN, USA) in a patient with a previous history of percutaneous ...

Increased myocardial energy expenditure in cardiac syndrome X: More work, more pain.
CONCLUSION: This study demonstrated that MEE was greater in CSX patients compared with a control group. Increased MEE was determined to be an independent predictor of CSX. DTS was inversely correlated with MEE. Increased MEE may have a crucial role in CSX pathophysiology. PMID: 30204135 [PubMed - in process]

The association between severe mitral stenosis and the size of the aortic root and the ascending thoracic aorta.
CONCLUSION: The presence of significant stenosis at the level of the mitral valve is associated with a smaller diameter in the aortic annulus. It is yet to be clarified whether this phenomenon occurs due to chronic, long-standing, low stroke volume or involvement of the aortic annulus in the fibrotic process of mitral disease. Association of cardiac adaptations with NT-proBNP levels after percutaneous closure of atrial septal defect.
CONCLUSION: Percutaneous ASD closure can lead to both early and sustained changes in cardiac anatomy and function involving both sides of the heart. The NT-proBNP level had increased at 24 hours post procedure, and was also notably increased 30 days after the percutaneous ASD closure, which is associated with increased LV diameter and volume. Current clinician perspective on non-vitamin K antagonist oral anticoagulant use in challenging clinical cases.
CONCLUSION: This expert consensus of cardiologists aimed to define management strategies for challenging cases with patients who underrepresented in pivotal trials using case examples from their routine practice. Although strong evidence is lacking, exploratory subgroup analysis of phase III pivotal trials partially informs the management of these patients. Clinical trials with higher external validity are needed to clarify areas of uncertainty. The lack of clear evidence about complex AF cases has pushed clinicians to manage patients based on clinical experience, including rare situations of off-label prescriptions. ...

Comparison of clinical features and conventional echocardiographic characteristics of patients with heart failure with mid-range ejection fraction with and without interatrial block.
CONCLUSION: To the best of our knowledge, the present study is the first to evaluate the clinical relevance of IAB in HFmrEF. Adding this simple ECG marker to the clinical evaluation could add significantly to the management of HFmrEF. IAB can be used to identify high-risk HFmrEF patients, as well as to guide follow-up and appropriate treatment. PMID: 33350397 [PubMed - as supplied by publisher]

Fabry disease in cardiology: Diagnosis and therapeutic approaches
Turk Kardiyol Dern Ars. 2020 Dec;48(Suppl 2):1-42. doi: 10.5543/tkda.2020.38262.ABSTRACTFabry disease is a rare, progressive, X-linked inherited storage disorder due to absent or deficient of lysosomal alfa galactosidase A activity. Deficient activity of alfa-galactosidase A results in progressive accumulation of globotriaosylceramide in a variety of tissues and organs including myocardium, kidney and nerve system. This disorder predominantly affects males; however, female heterozygotes may also be affected with a less severe clinical picture. Classic Fabry disease is usually diagnosed in early age of childhood because of ...
