Pre-travel advice for the Hungarian pilgrims of El Camino. Travel risks and instructions.
This study could be a useful guideline for pre-travel advice for El Camino pilgrims as only limited information can be gained from the Pilgrims' Reception Office (PRO) in Santiago de Compostela. Some information was collected from the relevant pages of the internet. Therefore the author could draw limited consequences from the statistical data. Yet, the analysis of the medical causes of the deceased pilgrims told more about the hazards of 'The Way'. Although the number of female and male pilgrims was equal in the statistical data, yet the mortality rate of the male pilgrims is much more higher than that of the female pilgr...

Pathophysiology and current clinical approach of ovarian hyperstimulation syndrome.
Abstract During assisted reproduction technologies, controlled hyperstimulation of the ovaries occurs. Ovarian hyperstimulation syndrome is an excessive overreaction of the ovaries complicating pharmacological ovulation induction. Rarely other causes, such as the mutation of the follicle-stimulating hormone receptor may also be in the background. Ovarian hyperstimulation syndrome is clinically characterized by a massive ovarian enlargement associated with an acute third-space fluid shift responsible for the development of ascites, and sometimes pleural or pericardial effusion. Associated arterial or venous thrombo...

Stroke prevention in the elderly: effectiveness and safety of edoxaban in elderly patients according to the results of the ENGAGE AF study.
Abstract Atrial fibrillation is the most common clinically relevant arrhythmia frequently causing systemic thromboembolic events. Traditionally vitamin K antagonists had been used for decades to prevent these events. The emerging of the new direct anticoagulants has revolutionized this treatment and a gradual growth and extensive spread of usage is expected. The latest one approved in Hungary, edoxaban, is a factor Xa inhibitor. Once-daily administration and favourable safety profile are major benefits of this drug. In a large clinical study with a high number of patients it proved to be at least as effective as w...

Immunoadsorption in a patient with dilated cardiomyopathy. The first case in Hungary.
Abstract Dilated cardiomyopathy is the main cause of heart transplantation. The etiology is unknown in almost half of the cases. Many cardiac specific antibodies have been identified till now which can cause decreased cardiac function, ventricular tachycardia or sudden heart death. The prognosis of DCM is poor despite the development of medical treatment. Immunoadsorption is hopeful since, with the removal of antibodies, cardiac function and NYHA class can improve and LVAD/heart transplantation-free survival can be prolonged. At the University of Debrecen, Faculty of Medicine, Department of Internal Medicine, Divi...

The efficacy and safety of edoxaban in non-valvular atrial fibrillation.
Abstract The significantly increased incidence of stroke and systemic embolisation caused by atrial fibrillation can be prevented by adequately adjusted anticoagulant therapy. Vitamin K antagonists effectively decrease the risk of thromboembolic events but this effect is influenced by many factors. The development of the new direct oral anticoagulant drugs (DOAC) in the last few years provided new opportunities for us to choose the suitable anticoagulant therapy. According to the results of the ENGAGE AF-TIMI 48 and ENSURE-AF multicenter, randomized trials, edoxaban, the recently introduced DOAC is equally effecti...

The labyrinth of residual risk: reduction of the remaining lipid and inflammation risk in the prevention of atherosclerosis.
Abstract Since cardiovascular diseases are the main cause of mortality worldwide, the reduction of their risk is a crucial point of present-day medicine. It has been proven unequivocally that the administration of various treatments has a favorable effect on the frequency of cardiovascular events and on the atherosclerosis leading to them. Although systematic and guideline-driven administration of these drugs has led to a decrease in the incidence and mortality of vascular events, the leading position of this group of diseases in mortality and morbidity has not changed. That is why medicine, besides keeping up act...

Thoracic aortic stentgraft implantations in Hungary from 2012 to 2016.
Abstract Thoracic aortic endograft implantation has become a widespread procedure in recent years, yet no report is available about Hungarian outcomes. Examination of our results is crucial to define further treatment strategies. Analysis of perioperative data from Hungarian thoracic endograft implantations based on the experience of 5 years is presented. Our retrospective, multicentric study analysed voluntarily reported data from all Hungarian institutions where thoracic endograft implantations are performed. Information was collected from every procedure performed in 5 years. Between 2012 and 2016, 131 thoracic...

Periprosthetic joint infection caused by Rhodococcus equi. Case report.
We present the first case of periprosthetic joint infection caused by Rhodococcus equi. An 88-year-old male patient was referred to our clinic with a history of fever and right hip pain. The patient had multiple hip surgeries including total joint arthroplasty and revision for aseptic loosening on the right side. He was immunocompetent, but his additional medical history was remarkable for diabetes mellitus, diabetic nephropathy and stroke with hemiplegia resulting in immobilization. Radiography showed stable components, joint aspirate yielded Rhodococcus equi. Irrigation and debridement was proposed, but the patient refus...

The first use of iv. idarucizumab for dabigatran reversal in Hungary.
Abstract At present, the direct thrombin inhibitor dabigatran is the only one amongst the new direct anticoagulants which has an effective, specific reversal agent. The novel agent idarucizumab is a humanized, monoclonal antibody fragment binds to dabigatran within minutes thereby offers an opportunity to induce a safe, long-lasting reverse of the anticoagulant effects of dabigatran. The authors describe the first use of idarucizumab in Hungary (23. 05. 2016) in an old female patient with non-valvular paroxysmal atrial fibrillation of high stroke risk-score and renal dysfunction who was taking dabigatran (2 x 110 ...

Classical antihypertensive drugs: diuretics.
Abstract The diuretics are essential medicaments of antihypertensive therapy. They reduce blood pressure and cardiovascular events optimally. With increasing doses of thiazides and thiazide analogs do not come further powerful effect of reducing blood pressure or cardiovascular mortality and morbidity, but clearly elevate the side effects. Because of it, the minimum effective dose level and the fixed-dose combination therapy should be preferred. The use these drugs leads to especially positive outcome in elder patients, isolated systolic hypertension, heart failure, after stroke and in black population. Loop diure...

Lipids and cerebrovascular disease - New therapeutic options in lowering LDL-cholesterol.
Abstract Stroke is the third most common cause of death worldwide following myocardial infaction and malignancies, furthermore, its functional outcome is the worst of all conditions. Cholesterol, especially LDL-cholesterol plays a key role in the formation of atherosclerotic plaques. It has been verified recently that escalating incidence and mortality of cerebrovascular diseases are proportional to increased levels of LDL-cholesterol. Statin therapy undeniably reduces the risk of stroke, however other methods for decreasing lipid levels have not been proved significantly effective. Preventive effect of high-dose ...

Risk of thromboembolism in atrial fibrillation.
Abstract Atrial fibrillation is considered as one of the cardiovascular pandemics of our days due to its increasing prevalence and the significant burden on healthcare systems. Management, especially prevention of thromboembolism associated with the arrhythmia is still a challenge even with recently available treatment options. Herein, the author reviews the possibilities of risk stratification and stroke prevention, which are important to all medical professionals who potentially encounter patients with this arrhythmia. Orv. Hetil., 2016, 157(38), 1511-1515. PMID: 27640617 [PubMed - in process]
