Ischemic stroke (IS), a common neurological disease with a variety of causes which is manifested as atherosclerosis or gradual cholesterol deposition, has become a leading cause of death and a major cause of permanent disability in many developed and developing countries. Chronic low-grade inflammation and activation of the innate immune system are closely involved in the pathogenesis of IS [1–2]. Mounting evidences from in vivo and ex vivo studies showed that inflammatory cytokines markers play a vital role in the development of IS [3].

Diabetes mellitus and risk of ischemic stroke in patients with heart failure and no atrial fibrillation
The risk of ischemic stroke, systemic thromboembolism, and all-cause death among heart failure patients previously diagnosed with diabetes mellitus is poorly described. We evaluated the risk of these endpoints among heart failure patients without diagnosed atrial fibrillation according to the presence of diabetes mellitus.

Ischemic stroke (IS), a common multifactor neurological disease with a variety of etiologies affected by a number of genetic mutations and environmental factors, is manifested by atherosclerosis or gradual cholesterol deposition. Chronic low-grade inflammation and activation of the innate immune system are closely involved in the pathogenesis of IS [1–2]. Cross-sectional studies have provided support for the hypothesis that chronic subclinical inflammation may be associated with atherosclerosis and precede the progression of clinically overt IS [3–4].

Details on methods and further thoughts on P-wave indices in stroke
We thank Dr. Chhabra for showing interest in our research. Earlier, Dr. Chhabra's group have found an association between interatrial block and embolic stroke, as well as with mesenteric ischemia [1,2]. In their studies, the method of measuring P-waves was manual and of high accuracy, using a magnifying graticule [1,2,5]. They did, however, only consider the longest P-wave duration in any single lead, not all leads simultaneously. In our study, we relied on measurements performed by the ECG device, which is considering all 12 leads simultaneously [3,4].

Hemorrhagic Transformation of Acute Ischemic Stroke is limited in Hypertensive Patients with Cardiac Hypertrophy
It has been clearly demonstrated that hypertension and one of its main evoked effects, cardiac hypertrophy, are independent risk factors for ischemic stroke. However, the ischemic brain lesions can further be affected by a second wave of injury characterized by hemorrhagic transformation (HT) of the primary ischemic lesion, which significantly aggravates the clinical outcome. So far, the risk factors that could affect such a transition in hypertensive patients are still unclear.

To improve survival and reduce morbidity, there are several international guidelines detailing tools to identify patients requiring treatment and direct best practices for medical, interventional and surgical therapies. Some of the more commonly used guidelines include the European Society of Cardiology's (ESC) 2012 update [2]; the American Heart Association (AHA), American College of Cardiology (ACC) and Heart Rhythm Society's (HRS) 2014 guideline [3]; and the Canadian Cardiovascular Society's (CCS) 2014 guidelines [1].
