189 Unexpected, Wanted and Kept: Management of a Pregnancy With Severe LV Dysfunction, Re-Do Mechanical Mitral Valve and Recurrent Arrythmias
We present the multidisciplinary management of a 31YO women with an unexpected pregnancy. Her background included congenital ASD and VSD, surgical repair of PDA and severe aortic coarctation at 2 weeks. Resection of sub-aortic membrane and repair of anterior mitral valve prolapse at 7 years. First mitral valve replacement (MVR) at 16 years of age (tissue valve), second MVR (mechanical) at 27 after presenting with a stroke due to valve thrombosis. LVEF 25% and severely dilated.

Distal arch aneurysm repair using left subclavian artery transposition with stented elephant trunk in the hybrid repair era
Hybrid aortic arch repair is an invasive approach to the surgical management of distal aortic arch aneurysm. The complications associated with hybrid aortic arch repair, such as stroke and endoleaks, are not uncommon and late reintervention is frequent. We retrospectively reviewed our experience of distal aortic arch aneurysm repair using the stented elephant trunk procedure with left subclavian artery (LSCA)-left common carotid artery (LCCA) transposition in the hybrid repair era.

New Onset Atrial Fibrillation Is Associated with Elevated Galactic-3 Levels
Background: Atrial fibrillation (AF) is associated with increased risk of stroke and cardiac mortality. Atrial remodelling and fibrosis have been suggested to participate in the pathogenesis of AF. Galectin-3 is a secreted protein that has regulatory roles in inflammation, fibrosis and tissue repair. Clinical studies have reported increased circulating galectin 3 levels and cardiac remodelling. Galectin-3 levels have been shown to predict atrial remodelling and incidence of AF. In the current investigation, we tested the hypothesis that Galectin-3 release occurs selectively in the acute phase of AF.
