Off-hour presentation and outcomes in patients with acute ischemic stroke: A systematic review and meta-analysis.
DISCUSSION: The evidence suggests that patients with acute ischemic stroke presenting during off-hours have higher short-term mortality and greater disability at discharge. PMID: 24721584 [PubMed - as supplied by publisher]

Proprotein convertase subtilisin kexin 9 (PCSK9) inhibitors to treat hypercholesterolemia: Effect on stroke risk.
CONCLUSION: Although a benefit towards reduction of cardiovascular events in the overall has been documented, longer exposure is warranted to be able to evaluate the effect on stroke risk. PMID: 27363304 [PubMed - as supplied by publisher]

Risk of stroke in CYP2C19 LoF polymorphism carrier coronary artery disease patients undergoing clopidogrel therapy: An ethnicity-based updated meta-analysis
CONCLUSION: Our meta-analysis confirmed that the presence of CYP2C19 LoF alleles increases the risk of stroke and composite events recurrence in the worldwide population, especially in Asians undergoing clopidogrel treatment. Alternative antiplatelet therapy should be investigated thoroughly for the intermediate and poor metabolizers.PMID:34092486 | DOI:10.1016/j.ejim.2021.05.022

Extended venous thromboprophylaxis in patients hospitalized for acute ischemic stroke: A systematic review and meta-analysis
CONCLUSION: In patients hospitalized for acute ischemic stroke, the net clinical benefit may favor extended venous thromboprophylaxis for four to five weeks over standard thromboprophylaxis.PMID:34649783 | DOI:10.1016/j.ejim.2021.09.016

Impact of gender-age interaction on the outcome of ischemic stroke in an Italian cohort of patients treated according to a standardized clinical pathway.
CONCLUSIONS: Our study confirms the excess risk of disability after stroke in women, although it is mostly explained by the occurrence of the most severe clinical syndromes. As for mortality, female gender seems to play a protective role, at least in the short-term and in younger patients. PMID: 23998857 [PubMed - as supplied by publisher]

Sepsis-driven atrial fibrillation and ischaemic stroke. Is there enough evidence to recommend anticoagulation?
Eur J Intern Med. 2021 Nov 8:S0953-6205(21)00361-7. doi: 10.1016/j.ejim.2021.10.022. Online ahead of print.ABSTRACTSepsis can lead to cardiac arrhythmias, of which the most common is atrial fibrillation (AF). Sepsis is associated with up to a six-fold higher risk of developing AF, where it occurs most commonly in the first 3 days of hospital admission. In many patients, AF detected during sepsis is the first documented episode of AF, either as an unmasking of sub-clinical AF or as a newly developed arrhythmia. In the short term, sepsis that is complicated by AF leads to longer hospital stays and an increased risk of inpati...

The association between direct oral anticoagulant concentration upon acute stroke and stroke outcome
CONCLUSION: Among DOAC users who developed IS, low drug concentrations at hospital presentation predicted poor outcomes.PMID:36990874 | DOI:10.1016/j.ejim.2023.03.023

Newer glomerular filtration rate estimating equations for the full age spectrum based on serum creatinine and cystatin C in predicting mortality in patients with ischemic stroke.
CONCLUSIONS: In patients with an ischemic stroke, estimated GFR based on serum cystatin (FAScysC and CKD-EPIcysC) was a better predictor of all-cause and cardiovascular mortality than estimated GFR based on serum creatinine. PMID: 29429860 [PubMed - as supplied by publisher]

Left atrial diameter thresholds and new incident atrial fibrillation in embolic stroke of undetermined source.
CONCLUSIONS: The LAD threshold of 40 mm has the best prognostic performance among other LAD values to predict new incident AF after ESUS. The diagnostic yield of prolonged cardiac rhythm monitoring in patients with LAD ≤ 40 mm seems low; therefore, such patients may have lower priority for prolonged cardiac monitoring. Stroke prophylaxis in high-risk patients with atrial fibrillation: Rhythm vs. rate control strategy.
CONCLUSIONS: Anti-thrombotic prophylaxis in CHADS₂ ≥2 patients is different in subjects assigned to the Rhythm vs. the Rate control strategy, as well as reported reasons for warfarin non use. GPs do not know why warfarin is not used in a large percentage of cases, mainly in the rhythm control strategy group. Improving efforts should probably be differently tailored for patients assigned to the "rhythm" or the "rate" control strategy. PMID: 23474251 [PubMed - as supplied by publisher]

Endovascular stroke therapy.
This article reviews the available endovascular therapeutic options and their evidence-base. SUMMARY: Initial studies demonstrated that endovascular treatment of ischaemic stroke with intra-arterial thrombolysis and/or the use of clot-retrieval, thrombus aspiration and stent-retriever devices produced early recanalisation and reperfusion and improved neurological outcome. More recent randomised trials, however, have failed to show translation of recanalisation into successful clinical outcome with 'time to treatment' proving crucial. In this rapidly evolving field, combined therapy incorporating intravenous and intra-...
