Extracranial carotid plaque hemorrhage predicts ipsilateral stroke recurrence in patients with carotid atherosclerosis - a study based on high-resolution vessel wall imaging MRI
CONCLUSIONS: Carotid intraplaque hemorrhage is strongly associated with recurrent ischemic events and could predict recurrent ipsilateral stroke.PMID:35764942 | DOI:10.1186/s12883-022-02758-3

Carotid Plaque Characteristics Predict Recurrent Ischemic Stroke and TIA: The PARISK (Plaque At RISK) Study
CONCLUSIONS: IPH and total plaque volume are independent risk factors for recurrent ipsilateral ischemic stroke or TIA in patients with mild-to-moderate carotid stenosis. These plaque characteristics improve current decision making. Validation studies to implement plaque characteristics in clinical scoring tools are needed. (PARISK: Validation of Imaging Techniques [PARISK]; NCT01208025).PMID:36202450 | DOI:10.1016/j.jcmg.2022.04.003

A comparative effectiveness study of carotid intervention for long-term stroke prevention in patients with severe asymptomatic stenosis from a large integrated health system
CONCLUSIONS: In this contemporary cohort study of patients with ACS utilizing rigorous analytic methodology, CEA appears to have a small but statistically significant effect on stroke prevention out to 8 years. Further study is needed to appropriately select the subset of patients most likely to benefit from intervention.PMID:37406943 | DOI:10.1016/j.jvs.2023.06.024

Management of extracranial carotid artery stenosis during endovascular treatment for acute ischaemic stroke: results from the MR CLEAN Registry
CONCLUSIONS: Functional outcomes were comparable after EVT with and without CAS. CAS during EVT might be a feasible option to treat the extracranial ICA stenosis but randomised studies are warranted to prove non-inferiority or superiority.PMID:36572506 | DOI:10.1136/svn-2022-001891

Effectiveness of an individualized home-based physical activity program in surgery-free non-endarterectomized asymptomatic stroke patients: a study protocol for the PACAPh interventional randomized trial
This study has been approved by the National Ethics Committee (IDRCB:2019-A01543-54/SI:19.06.21.40640). ClinicalTrials.gov NCT04053166.PMID:35164816 | DOI:10.1186/s13063-022-06061-x

Interleukin-6 Predicts Carotid Plaque Severity, Vulnerability, and Progression
CONCLUSIONS: Circulating IL-6 predicts carotid plaque severity, vulnerability, and progression. The 2.0 pg/mL cutoff could facilitate the selection of individuals that would benefit from anti-IL-6 drugs for stroke prevention.PMID:35713008 | DOI:10.1161/CIRCRESAHA.122.320877

Early postoperative outcome after carotid artery endarterectomy with bifurcation advancement
Conclusions: CEA with bifurcation advancement provides comparable perioperative results, focusing on combined stroke and mortality, but seems to have some technical drawbacks, which may lead to more frequent local neurological complications.PMID:37170543 | DOI:10.1024/0301-1526/a001074

Multimodal recanalization for subacute symptomatic internal carotid artery occlusion due to atherosclerosis: Outcomes and notes on hybrid techniques
CONCLUSION: Multimodal recanalization techniques performed in a hybrid operation theater can safely achieve high recanalization rates in atherosclerotic ICAO patients in the subacute phase, which can be beneficial for recovering CBP and preventing stroke.PMID:35247538 | DOI:10.1016/j.avsg.2022.01.022

One-Year Outcomes After Transcarotid Artery Revascularization (TCAR) in the ROADSTER 2 TRIAL
CONCLUSIONS: In patients with high risk factors, TCAR yields high technical success with a low stroke and death rate at one year. Further comparative studies to CEA are warranted.PMID:35381327 | DOI:10.1016/j.jvs.2022.03.872

Carotid Plaque Imaging: Strategies beyond Stenosis
Ann Indian Acad Neurol. 2022 Jan-Feb;25(1):11-14. doi: 10.4103/aian.aian_483_21. Epub 2021 Aug 23.ABSTRACTAtherosclerosis at the carotid bifurcation in the neck is common and the result of turbulent hemodynamics in the bulb. Carotid bulb plaques cause disabling strokes. Carotid vessel wall imaging characterizes the atherosclerotic plaque, documents disease progression, and assesses lesion severity by plaque morphology in addition to the degree of stenosis. This in turn optimizes treatment selection for the individual patient. The aim of vessel wall imaging in patients of stroke is to distinguish stable plaques that may be ...
