Combined carotid endarterectomy and retrograde stenting of the supra-aortic trunk: Does cervical block offers advantages?
Atherosclerosis of the carotid artery is a major source of stroke. In some cases atherosclerosis occurs at several positions within the carotid artery. Carotid endarterectomy (CEA) in combination with retrograde balloon angioplasty and stenting of a brachiocephalic or common carotid artery stenosis has been described as efficacious and save procedure to prevent stroke in these cases. The aim of this study was to analyse the impact of anaesthetic techniques on hemodynamic factors, operation time, duration of clamping and post-operative pain.

Prospective Randomized Trial of ACUSEAL Versus Vascu-Guard Patching in Carotid Endarterectomy
Conclusions: Although CEA patching with ACUSEAL versus Vascu-Guard differed in hemostasis time, the frequency of reexploration for neck hematomas was more frequent in the pericardial patch group; however, only 1 patient had documented suture line bleeding and the surgical reexploration rate is not likely to be patch related. There were not any significant differences in perioperative/late neurologic events and late restenosis in the 2 groups.

Use of the Vascu-Guard Bovine Pericardium Patch for Arteriotomy Closure in Carotid Endarterectomy. Early and Long-Term Results
Conclusions: This series shows the efficacy, safety, and durability of CEA with BP patch closure, in both the short and long term. BP combines the advantages of vein and synthetic patches, representing a suitable option when performing CEA.

Longitudinal Evaluation of Neurobehavioral Outcomes After Carotid Revascularization
Conclusions: The relationship between carotid revascularization and cognitive function is complex, and cognitive deficits may occur in the presence of increased cerebral perfusion without detectable embolization.

Postoperative Cardiac Damage After Standardized Carotid Endarterectomy Procedures in Low- and High-Risk Patients
Conclusions: Carotid endarterectomy is followed by an increase in cTn-I value>0.5 ng/mL in 14% of all cases, although symptomatic cardiac ischemia is very low. However, high-risk patients as defined by the SAPPHIRE criteria do not show an increased risk of cardiac damage compared to low-risk patients. Larger studies using cTn-I as a marker of postoperative cardiac damage, after carotid endarterectomy or stenting, are needed.
