Carotid endarterectomy or stenting in octogenarians in a monocentric experience
Carotid artery stenting (CAS) has emerged as an alternative to carotid endarterectomy (CEA) in patients at high risk for complications from surgery. The very elderly (>or=80-year-old) are one subgroup of patients identified as being at increased risk for carotid surgery. However, there is concern that the very elderly are also at increased risk for complications of CAS. A stroke and death rate of 12% was reported in very elderly patients during the roll-in phase of Carotid Revascularization Endarterectomy versus Stent Trial (CREST).

The Influence of Collagen Impregnation of a Knitted Dacron Patch Routinely Used in Carotid Endarterectomy
In selected populations, carotid endarterectomy (CEA) reduces long-term stroke risk. Studies have shown increased risk of restenosis with use of a collagen-impregnated Dacron patch compared to a PTFE patch. There is concern that collagen-impregnation may initiate thrombosis or increase restenosis due to platelet activation. We performed a retrospective analysis of our CEA experience with routine patching using knitted Dacron patches with (Hemashield) and without (Sauvage) collagen-impregnation.

Do Patients Understand Their Cardiovascular Risk Factors and Impact On Complications?
While considerable research exists about cardiovascular disease, there is limited data regarding patients’ understanding of their own cardiovascular disease risk factors and cardiovascular complications. We set out to assess patients understanding of their own cardiovascular risk factors and complications of cardiovascular disease. In particular we were curious as to patients’ fears of stroke and myocardial infarction (MI) related to recent publications regarding risks related to carotid interventions.

Increased Duration of Operating Time for Carotid Endarterectomy is Associated with Increased Mortality
Carotid endarterectomy is the gold standard operation to prevent stroke in patients with symptomatic carotid artery stenosis and asymptomatic high-grade carotid artery stenosis. Longer operative times for different operations have been shown to affect the outcomes adversely. The purpose of this study was to determine the incidence of post-operative complications after carotid endarterectomy, and their relation to the operative times.

Barostim implantation with ipsilateral carotid endarterectomy as a one-stage procedure
Hypertension (HTN) remains a significant public health problem with substantial patient morbidity, mortality, and cost to the health care system despite advances in treatment (1). Clinical trials have demonstrated significant and lasting reductions in arterial pressure from baroreflex activation therapy (BAT), which functions by electrical stimulation of the carotid sinus using the Barostim neo™ system in patients with resistant HTN (2, 3). Significant carotid atherosclerosis, however, has been a contraindication for ipsilateral implantation due to increased risk of periprocedural stroke and uncertain antihypertensive efficacy.

Recognizing the applicability of a standardized care process to vascular emergencies, Dr. Michael Dalsing of Indiana University –Methodist Hospital led his academic faculty and hospital in the development of a statewide referral and rapid response system for vascular emergencies in 2009.
