Translesional pressure gradient and leptomeningeal collateral status in symptomatic middle cerebral artery stenosis
ConclusionsOur study suggested a correlation between translesional pressure gradient and maturation of leptomeningeal collaterals in intracranial atherosclerotic disease. Further studies with more exquisite and dynamic monitoring of cerebral hemodynamics and LMC evolution are needed to verify the current findings.This article is protected by copyright. Utility of digital subtraction angiography ‐based collateral evaluation in medically treated acute symptomatic basilar artery stenosis
ConclusionsThe DSA‐based collateral scoring of the BA large branches might predict long‐term outcome in medically treated symptomatic basilar artery stenosis with mild initial severity. Evaluation of BA collateral perfusion status might be useful to determine appropriate treatment strategies.

Stroke etiology and collaterals: atheroembolic strokes have greater collateral recruitment than cardioembolic strokes
ConclusionsAtheroembolic strokes seem to be associated with better collateral flow compared to cardioembolic strokes. This may in part explain the worse outcomes of AF‐related stroke.

Added value of multimodal computed tomography imaging: analysis of 1994 acute ischaemic strokes
ConclusionOur study shows an association of MCTI use with lower adjusted 12‐month mortality, better identification of the stroke mechanism and no signs of harm.

Impact of ASPECTS on computed tomography angiography source images on outcome after thrombolysis or endovascular therapy in large vessel occlusions
ConclusionPatients with extensive early ischaemic changes on CT scans (SI‐ ASPECTS ≤5) might not profit from ET. The impact of ET on outcome in patients with moderately favorable SI‐ASPECTS should be addressed in further trials.

Impact of thrombolysis in acute ischaemic stroke without occlusion: an observational comparative study
ConclusionsOur study suggests that AIS patients without visible arterial occlusion treated with IV‐rtPA may have no better outcome at 3 months than those untreated. However, only a randomized controlled trial would provide a definitive answer about the impact of rtPA in acute stroke patients without occlusion. Until then, these patients should be treated by rtPA as recommended.

ConclusionsThe decision to thrombolyse or perform an LP in HaNDL patients mimicking a stroke is difficult in the acute setting. Perfusion CT can provide misleading results and CTA may be useful in ruling out occlusion of a cerebral vessel.

Radiological imaging in acute ischaemic stroke
Patients who suffer acute ischaemic stroke can be treated with thrombolysis if therapy is initiated early. Radiological evaluation of the intracranial tissue before such therapy can be given is mandatory. In this review current radiological diagnostic strategies are discussed for this patient group. The relationship between the pre‐admission CHA2DS2‐VASc score and proximal artery occlusion in patients with acute stroke and atrial fibrillation
ConclusionThe CHA2DS2‐VASc score was associated with PAO in acute ischaemic stroke patients with AF. The CHA2DS2‐VASc score may be correlated with the NIHSS score through the presence of PAO and with poor functional outcome independently of PAO.

Symptomatic intracranial haemorrhage after thrombolysis with adjuvant anticoagulation in basilar artery occlusion
ConclusionsThe risk of sICH was largely determined by extension of ischaemic changes on admission computed tomography. Clinically relevantly, also higher post‐thrombolytic SBP as described earlier and lower perithrombolytic platelet counts do increase the risk, a finding requiring confirmation in other patient series.
