Secondary prevention in patients with vascular disease. A population based study on the underuse of recommended medications
Conclusions Our findings underline the fact that the underuse of secondary preventive therapies is common in patients with recurrent vascular events, especially those with previous cerebral ischaemia. This underuse could be targeted to reduce recurrent vascular events.

Alterations in structural and haemodynamic cerebrovascular measures in late onset epilepsy
Conclusions Our findings indicate differences in both structural and haemodynamic measures of CVD between LOE patients and HC, even in the absence of clinical CVD. We observed trends towards longer baseline tA and reduced tA in response to hypercapnia in LOE patients, and significant regional tA differences. Long-term prognosis after intracerebral haemorrhage: systematic review and meta-analysis
Conclusions Less than a half of patients with ICH survive 1 year and less than a third survive 5 years. Risks of recurrent ICH and ischaemic stroke after ICH appear similar after ICH, provoking uncertainties about the use of antithrombotic drugs.

Basilar extension and posterior inferior cerebellar artery involvement as risk factors for progression of the unruptured spontaneous intradural vertebral artery dissection
Conclusions Thus, some vertebrobasilar morphologies might be markers of the progression of unruptured siVAD. Although all unruptured siVAD patients should be closely monitored, those with basilar extension and posterior inferior cerebellar artery involvement should perhaps be more carefully followed than those without such morphologies.

Cortical susceptibility-weighted imaging hypointensity after stroke-like episode in MELAS

Spinal cervical extradural haematoma presenting as stroke
Conclusion With the increasing prevalence of thrombolysis for the treatment of acute stroke, spinal epidural haematoma is an important differential diagnosis to consider in patients manifesting acute-onset limb weakness due to the potentially catastrophic consequences of thrombolysing these patients erroneously.

Outcome of patients with occlusions of the internal carotid artery or the main stem of the middle cerebral artery with NIHSS score of less than 5: comparison between thrombolysed and non-thrombolysed patients
Conclusions Non-thrombolysed patients with mild deficits and large vessel occlusion deteriorated significantly more often within 3 months than thrombolysed patients. Symptomatic intracerebral haemorrhages occurred in less than 5% of patients in both groups. These data suggest that thrombolysis is safe and effective in these patients. Therefore, randomised trials in patients with large vessel occlusions and mild or rapidly improving symptoms are needed.

A case of atraumatic convexal subarachnoid haemorrhage due to unusual cause
Conclusions This case shows that ICA stenosis can cause convexal SAH and it should therefore be added to the differential diagnosis of atraumatic SAH. The association is under recognised and can be missed due to the unusual location of the blood and failure to image the extra cranial carotid arteries.

The zebra sign
A 65 year old female was transferred to the Hyperacute Stroke Unit after a routine L5/S1 laminectomy. She suffered a small dural tear. 3 hours post procedure, her GCS dropped to 3. CT scan showed a bi-cerebellar haemorrhage and MRI brain the next day revealed a convexity subarachoid haemorrhage. Imaging was reviewed with neuroradiologists and neurosurgeons. They recognised this as a typical case of post-dural puncture remote cerebellar haemorrhage. In surgical literature there are many cases reported since 1981 of "the Zebra sign".
