Abstract Stroke causes 5.7 million deaths annually. This ranks stroke as the second most common cause of death and, additionally, it is a major cause of disability. Prolonged impairment of deglutition in supratentorial ischaemic stroke: the predictive value of Parramatta Hospitals' Assessment of Dysphagia.
CONCLUSIONS: In a selected population at risk of aspiration, the PHAD with a threshold of 70 assessed in the second week after stroke onset may be a valuable tool to predict prolonged impairment of deglutition for another 4 weeks and to guide the decision about switching from NG to PEG tube feeding after supratentorial ischaemic stroke. Stroke management in a Swiss community hospital - in close collaboration with a stroke centre
CONCLUSION: Our findings support further education of the population in recognition of stroke symptoms and assessment of cardiovascular risk factors according to guidelines. Cost-effectiveness of dabigatran for stroke prevention in atrial fibrillation in Switzerland.
CONCLUSIONS: Dabigatran can be considered cost-effective in comparison with vitamin K antagonists in the Swiss context. The higher drug cost of dabigatran is compensated by savings in INR monitoring, lower cost of clinical events and QALY-gains. PMID: 23300013 [PubMed - in process]

Relevance of the cerebral collateral circulation in ischaemic stroke: time is brain, but collaterals set the pace.
Abstract Blood supply to the brain is secured by an extensive collateral circulation system, which can be divided into primary routes, i.e., the Circle of Willis, and secondary routes, e.g., collaterals from the external to the internal carotid artery and leptomeningeal collaterals. Collateral flow is the basis for acute stroke treatment, since neurones will only survive long enough to be rescued with reperfusion therapies if there is sufficient collateral flow. Left atrial appendage closure for prevention of cardioembolic events.
We describe the pathophysiology of the LAA with regard to stroke. Aside from the evidence and limitations of anticoagulation as the classical treatment paradigm for stroke prevention, devices and techniques for LAAC are outlined and the current clinical evidence with regard to efficacy and safety is reviewed. Anticoagulation use in perioperative atrial fibrillation after noncardiac surgery: a systematic review and meta-analysis
CONCLUSION: Anticoagulation is associated with a reduced risk of stroke and death but an increased risk of bleeding. The quality of the evidence is very poor. Management of aneurysmal subarachnoid haemorrhage.
Abstract Aneurysmal subarachnoid haemorrhage (aSAH) occurs as a result of rupture of an intracranial aneurysm and affects a younger population compared with ischaemic stroke or intracerebral haemorrhage. Although it makes up only about 5% of all cerebrovascular events, it accounts for over a quarter of the productive life-years lost to stroke. Atrial fibrillation for internists: current practice.
Abstract Atrial fibrillation (AF) has become a global epidemic and puts affected patients at high risk of adverse events. In this review we summarise the current evidence on risk factors and complications of AF, describe current treatment strategies, and outline new fields of research. Current evidence shows that hypertension and obesity are the two most important modifiable risk factors for the development of AF. Patients with AF face an increased stroke risk.
