Tofacitinib for the treatment of psoriasis and psoriatic arthritis.
This article aims to summarize the efficacy and safety data of tofacitinib for treatment of psoriatic disease. A comprehensive review of literature was conducted on the PubMed database using the search terms: "((tofacitinib) AND (psoriasis)) OR ((tofacitinib) AND (psoriatic arthritis))". Data from the pivotal clinical trials evaluating tofacitinib in the treatment of psoriatic disease were summarized. The Oral-treatment Psoriasis Trial (OPT) study series demonstrated that tofacitinib is efficacious in the treatment of plaque psoriasis with an acceptable safety profile. The OPT studies also demonstrated the non-inferiority ...

Measuring the horizontal and vertical growth rates of superficial spreading melanoma: a pilot study with sequential digital dermoscopy.
CONCLUSIONS: A small proportion of SSMs with a slow horizontal growth could have a precocious vertical growth. Other larger studies are needed to confirm this observation. PMID: 32129054 [PubMed - as supplied by publisher]

Patient-reported outcomes for keloids: a systematic review.
CONCLUSIONS: Patient-reported outcomes provide valuable insights into the perceptions, treatment goals, and quality of life of keloid patients. Development of more patient-reported outcome measures specific to keloids, especially those that incorporate both scar and quality of life assessments, may help refine our current understanding of keloid management. Wound odor: current methods of treatment and need for objective measures.
Authors: Darwin ES, Thaler ER, Lev-Tov HA Abstract Chronic wounds are an enormous burden to society, costing billions of dollars annually in the US alone. Despite the extensive research into methods to heal chronic wounds, many remain unhealed for months to years. There is a need to focus on patient reported outcomes to improve quality of life in patients with non-healing wounds. Wound odor has a significant impact on patient quality of life; however, relatively little information is available on the management of wound odor. We review the current data available on wound odor and discuss the need for standardized o...

Long-term efficacy data for daylight-PDT.
CONCLUSIONS: DL-PDT is a very promising treatment for mild to moderate AKs of the face and scalp. Efficacy outcomes of DL-PDT are similar to those of c-PDT in the short-term. Additional studies are required to increase our knowledge on DL-PDT long-term efficacy, as limited data are currently available. PMID: 29683285 [PubMed - as supplied by publisher]

Isotretinoin: the ups are just as troubling as the downs.
CONCLUSIONS: Isotretinoin may cause mood instability in both directions-depression and mania-- especially in a predisposed population. With this in mind, we urge clinicians prescribing isotretinoin to focus on all psychiatric symptoms (not just depression) including mania, mixed mood, and psychosis, paying particular attention to individuals who have a personal or family history of psychiatric disease. Clinical research in dermatology: resources and activities associated with a higher scientific productivity.
CONCLUSIONS: We have demonstrated that some factors are associated with a higher scientific productivity. Residency program, more research staff, clinical workload redistribution and research motivation/initiatives are key strategies that could improve scientific productivity of a center. Atopic dermatitis treatment: what's new on the horizon?
Authors: Panarese F, Auriemma M, Carbone A, Amerio P Abstract Atopic dermatitis (AD) is an inflammatory disorder of the skin characterized by an impaired immune response and skin barrier function. It is very frequent in adult population being present in up to 10% of population. Quality of life is often reduced in AD patients due to disease burden and symptoms like itch. AD is also frequently associated with psychological diseases such as anxiety or depression. Due to its chronic nature and severity of presentation AD often may not respond to topical treatment and requires systemic treatments which can be associated...

The association between serum vitamin D levels and urticaria: a meta-analysis of observational studies.
CONCLUSIONS: The study demonstrated that patients with urticaria had a lower Vitamin D level than healthy controls. Vitamin D deficiency rather than insufficiency has a positive relationship with urticaria. However, larger-scale studies over alonger duration of treatment are needed to confirm this conclusion. Thick melanoma in Tuscany.
CONCLUSIONS: If we want to obtain better results on diagnosis of skin melanoma we have to think a new strategy. At least to think over the educational messages discriminating people more at risk of incidence of melanoma from people more at risk to die from melanoma, and to renewed active involvement of the Gen- eral Practitioners . Staging and follow-up of cutaneous melanoma patients.
CONCLUSIONS: Based on a universally-accepted staging system different surveillance procedures have been developed, which may be mainly classified in two groups: low- and high-intensity strategies. PMID: 28195451 [PubMed - as supplied by publisher]

Recurrent herpes labialis and HSV-1 herpes genitalis: which is the link?
CONCLUSIONS: We can speculate about a protective role for RHL against the clinical appearance of HSV-1 HG. The clinical usefulness of our study involved especially the counseling in serodiscordant couples. The presence of HSV-1 antibodies in asymptomatic sexual partners does appear protective for HG manifestation only in presence of RHL history. PMID: 28181784 [PubMed - as supplied by publisher]

Systematic review of the relationship between psoriasis and metabolic syndrome: a critical appraisal.
CONCLUSIONS: Psoriasis patients have a greater prevalence of metabolic syndrome as well as its individual components when compared to the general population. The odds of metabolic syndrome and its components are higher with increased psoriasis disease severity. Prospective studies are needed to better understand the contribution of psoriasis in the development of metabolic syndrome. PMID: 27589483 [PubMed - as supplied by publisher]

Medication patterns in chronic spontaneous urticaria: results from a nationwide investigation in the primary care setting in Italy.
CONCLUSIONS: The results of our study have shown that H1-antihistamines were the most prescribed drugs to treat CSU, reflecting their role as the mainstay of CSU treatment. Systemic corticosteroids continue to be frequently prescribed for CSU in clinical practice. Alternative drugs were used only in very few patients, suggesting the possibility that severe refractory cases of CSU are undertreated in clinical practice. Non-melanoma skin cancer of the head and neck: the aid of reflectance confocal microscopy for the accurate diagnosis and management.
CONCLUSION: RCM allows tissue imaging in-vivo contributing to a more accurate diagnosis of NMSC of the head and neck, sparing time for the patient and costs for the public health system. RCM can also be used for selection of the biopsy site and it is helpful in defining the surgical safety margins to keep during the excision of skin cancers.
