Surgical procedures in melanoma in 2020: recommended deep and lateral margins, indications for sentinel lymph node biopsy, and complete lymph node dissection.
CONCLUSIONS: Although the trend is to reduce progressively the recommended surgical margins, further evidence is needed to clarify its role in patients' survival. Sentinel lymph node biopsy is important for establishing a prognosis especially upon considering adjuvant therapy; complete lymph node dissection is only relevant for regional disease control. Analysis of key markers: IL-10/sHLA-G in psoriasis patients and the identification of 14-bp INDEL in the HLA-G gene.
Authors: Bieniek-Kobuszewska M, Owczarczyk-Saczonek A, Maciejewska-Radomska A, Wojtacha P, Placek W Abstract BACKGROUND: Psoriasis is a chronic inflammation resulting from interactions between immunological and genetic factors. An important tolerogenic role in this autoimmunological disease is played by HLA-G, which is modulated by IL-10. Therefore, this study (N=80) aimed to evaluate changes in the serum sHLA-G and IL-10 levels in active psoriasis vulgaris and in the early stages of treatment with Methotrexate (MTX) compared to healthy controls. The 14-bp INDEL of the HLA-G gene was evaluated to find possible asso...

Androgenetic alopecia in women and men: Italian guidelines adapted from European Dermatology Forum/European Academy of Dermatology and Venereology guidelines.
Authors: Alessandrini A, Starace M, D'''''Ovidio R, Villa L, Rossi A, Stan TR, Calzavara-Pinton P, Piraccini BM Abstract Androgenetic alopecia (AGA) is the most common form of alopecia, affecting up to 80% of men and 50% of women in the course of their life. AGA is caused by a progressive reduction in the diameter, length and pigmentation of the hair, resulting from the effects of the testosterone metabolite dihydrotestosterone (DHT) on androgen-sensitive hair follicles. Clinical presentation is different in men and women. Trichoscopy is used routinely in patients with androgenetic alopecia, for diagnosis and diffe...

Genetic mutations in primary malignant melanoma of the esophagus: case report and literature review.
We report a case of a 66 years old man with a PMME in the lower third of the esophagus. Analysis of c-kit, KRAS, NRAS and BRAF genes resulted negative for mutations. On the basis of a computerized (PuMed/Medline) bibliography search we retrieved a total of other 35 cases of PMME analyzed for genetic alterations in RAS, BRAF, and KIT. When we compared mutations frequency of PMME with those of other mucosal melanomas, it appeared that PMME are characterized by a relative higher percentage of NRAS mutations. PMME seem to show a specific pattern of genetic alterations suggesting that they could represent a distinct entity amon...

Immune serological diagnosis of pemphigus.
Authors: Solimani F, Meier K, Zimmer CL, Hashimoto T Abstract Pemphigus is a rare autoimmune blistering disease which manifests with painful erosions and blisters of the skin and mucosa. This disorder is caused by autoantibodies attacking desmosomal proteins, necessary for cell-cell contact stability and epidermal integrity. Desmoglein (Dsg) 1 and Dsg3 are the two major target antigens in pemphigus. Yet, many other target proteins, which have been described over the years, seem to be involved in the loss of epidermal integrity. Clinical examination, combined to serological advances and detection of targeted antigen...

Autoreactive T cells in pemphigus. Perpetrator and target.
Authors: Scarsella L, Pollmann R, Amber KT Abstract Pemphigus vulgaris (PV) is an autoimmune blistering disease, in which autoantibodies against epidermal cadherins, such as desmoglein (Dsg)1 and Dsg3, lead to the development of blisters and erosions on the skin and the mucous membranes. Autoreactive CD4+ T cells are essential for the induction and perpetuation of the disease by interaction with B cells producing autoantibodies. PV has a strong genetic association with certain human leucocyte antigen (HLA) alleles with being HLA-DRB1*04:02 and DQB1*05:03 the most prevalent in patients. Recently, genome-wide associa...

Artificial intelligence for melanoma diagnosis: a review of published studies until 2020.
Authors: Tschandl P Abstract Convolutional neural networks (CNN) have shown unprecedented accuracy in digital image analysis, which can be harnessed for melanoma recognition through automated evaluation of clinical and dermatoscopic images. In experimental studies, modern CNN architectures perform single image analysis at the level of dermatologists and domain-experts, also for multi-class predictions including a multitude of possible diagnoses. This may not necessarily translate to good clinical performance, and reliable randomised controlled prospective clinical trials for modern CNNs are essentially missing. Wea...
