Stroke Prevention in Symptomatic Large Artery Intracranial Atherosclerosis Practice Advisory: Report of the AAN Guideline Subcommittee
Neurology. 2022 Mar 22;98(12):486-498. doi: 10.1212/WNL.0000000000200030.ABSTRACTBACKGROUND AND OBJECTIVES: To review treatments for reducing the risk of recurrent stroke or death in patients with symptomatic intracranial atherosclerotic arterial stenosis (sICAS).METHODS: The development of this practice advisory followed the process outlined in the American Academy of Neurology Clinical Practice Guideline Process Manual, 2011 Edition, as amended. The systematic review included studies through November 2020. Recommendations were based on evidence, related evidence, principles of care, and inferences.MAJOR RECOMMENDATIONS: ...

Rescue Strategies in Anterior Circulation Stroke with Failed Mechanical Thrombectomy-A Retrospective Observational Study (RAFT)
CONCLUSIONS: Rescue stenting and/or angioplasty after failed MT is a safe and effective recanalization method for AIS in anterior circulation without increasing mortality or morbidity.PMID:35359559 | PMC:PMC8965932 | DOI:10.4103/aian.AIAN_87_21

Posterior Circulation Ischaemic Stroke
Am J Med Sci. 2022 Jan 29:S0002-9629(22)00030-1. doi: 10.1016/j.amjms.2021.10.027. Online ahead of print.ABSTRACTPosterior circulation ischaemic stroke (PCIS) is a disease of high burden. They account for 20-25% of all ischaemic strokes. However, it is relatively under-researched and requires more clinical attention, since it carries worse functional outcomes. Vertigo, visual disturbances and sensory/motor disturbances are found in PCIS. Large artery atherosclerosis and embolism are main causes of PCIS, while there is growing evidence that vertebrobasilar dolichoectasia is a key association. Hypertension is the commonest r...

Angiography-based hemodynamic features predict recurrent ischemic events after angioplasty and stenting of intracranial vertebrobasilar atherosclerotic stenosis
CONCLUSIONS: In patients with intracranial vertebrobasilar atherosclerosis treated with PTAS, preprocedural prolonged TTP of the target vessel and shortened trans-stenotic TTP difference were associated with stroke relapse. Q-DSA-defined hemodynamic parameters provided incremental predictive value over conventional parameters for stroke recurrence.CLINICAL RELEVANCE STATEMENT: Quantitative DSA analysis enables intuitive observation and semi-quantitative evaluation of peri-therapeutic cerebral blood flow. More importantly, quantitative DSA-defined hemodynamic parameters have the potential for risk stratification of patients...

Intracranial atherosclerotic stenosis: risk factors, diagnosis, and treatment
Lancet Neurol. 2022 Feb 7:S1474-4422(21)00376-8. doi: 10.1016/S1474-4422(21)00376-8. Online ahead of print.ABSTRACTIntracranial atherosclerotic stenosis (ICAS) is one of the most frequent causes of stroke worldwide and confers one of the greatest risks of recurrent stroke compared with other causes of stroke. Asymptomatic ICAS is increasingly recognised as a risk factor for silent brain infarctions and dementia, magnifying the global burden of ICAS. Although ICAS is a lumen-based diagnosis, newer diagnostic imaging techniques, such as high-resolution MRI, might help to identify high-risk population subgroups to test interv...

Questionnaire-based exposome-wide association studies (ExWAS) reveal expected and novel risk factors associated with cardiovascular outcomes in the Personalized Environment and Genes Study
In conclusion, using statistics and machine learning, these findings identify novel potential risk factors for CVD, enable hypothesis generation, provide insights into the complex relationships between risk factors and CVD, and highlight the importance of considering multiple exposures when examining CVD outcomes.PMID:35605674 | DOI:10.1016/j.envres.2022.113463

Balloon Angioplasty for Symptomatic Intracranial Artery Stenosis (BASIS): protocol of a prospective, multicentre, randomised, controlled trial
DISCUSSION: BASIS trail is the first RCT to compare the efficacy and safety of balloon angioplasty plus AMM to AMM alone in sICAS patients, which may provide an alternative perspective for treating sICAS.TRIAL REGISTRATION NUMBER: NCT03703635; https://www.CLINICALTRIALS: gov.PMID:37202152 | DOI:10.1136/svn-2022-002288

Angioplasty And stenting For symptomatic intracranial atherosclerotic disease: How I Do It
Interv Neuroradiol. 2022 Mar 24:15910199221090430. doi: 10.1177/15910199221090430. Online ahead of print.ABSTRACTIntracranial atherosclerotic disease (ICAD) is a common cause of acute ischemic stroke. ICAD has a high rate of recurrent ischemic stroke despite recommended adequate medical treatment. Endovascular treatment of ICAD has been controversial due to high periprocedural complication in historic randomized clinical trials (RCTs). Recently, a multicenter prospective study of stent placement in patients with high grade intracranial stenosis that evaluated safety and efficacy of Balloon-mounted stents (BMS) in symptomat...

Staged endovascular recanalization for symptomatic atherosclerotic non-acutely occluded internal carotid artery
CONCLUSIONS: This retrospective study found that staged endovascular recanalization for symptomatic atherosclerotic NAOICA may be feasible with an acceptable technical success rate and a low complication rate in the selected candidates.PMID:36883208 | DOI:10.1177/15910199231162666
