D-dimer and the risk of hypertension: The REasons for Geographic And Racial Differences in Stroke Cohort Study
CONCLUSION: D-dimer concentration reflects pathophysiology related to the development of hypertension. Specific mechanisms require further study and may involve adiposity.PMID:36760775 | PMC:PMC9903654 | DOI:10.1016/j.rpth.2022.100016

Stroke and myocardial infarction in patients with abdominal aortic aneurysms and new-onset atrial fibrillation
Conclusion Cardiovascular prognosis has improved in patients with prevalent AAA disease and new-onset AF in concordance with optimization of antithrombotic therapy over time. A diagnosis of AF conferred residual risk of stroke and myocardial infarction.PMID:36626930 | DOI:10.1055/a-2009-8954

Expression of microRNA Predicts Cardiovascular Events in Patients with Stable Coronary Artery Disease
CONCLUSIONS: Among patients with CAD, adding miR-223-3p expression to traditional cardiovascular risk factors may improve prediction of cardiovascular events, particularly ST. Clinical trials confirming these findings are warranted.PMID:36603835 | DOI:10.1055/s-0042-1760258

Severe left atrial spontaneous echo contrast in non-valvular atrial fibrillation: Clinical characteristics and impact on ischemic risk post-ablation
Conclusions Higher BNP, ESR, LAd/∆LVED were the independent predictors for severe SEC. Severe SEC were associated with higher peri-procedural and long-term ischemic risks. ESR and LAd/∆LVED, as indicators of hematological and hemodynamic change, seemed helpful in identifying NVAF patients prone to developing severe SEC.PMID:36402133 | DOI:10.1055/a-1983-0516

Alteplase or tenecteplase for thrombolysis in ischemic stroke: An illustrated review
Res Pract Thromb Haemost. 2022 Sep 20;6(6):e12795. doi: 10.1002/rth2.12795. eCollection 2022 Aug.ABSTRACTIntravenous thrombolysis is a standard of care treatment for patients with acute ischemic stroke. Tissue plasminogen activator (tPA) has been the main thrombolytic agent used since the publication of the seminal National Institutes of Neurological Disorders and Stroke trial in 1995. There is now mounting evidence to support the routine use of Tenecteplase (TNK) to treat acute ischemic stroke. TNK is a genetically modified tPA with higher fibrin specificity, longer half-life, and reduced systemic coagulopathy. In this il...

Hyperhomocysteinemia in cardiovascular diseases: revisiting observational studies and clinical trials
In conclusion, revisiting observational studies and clinical trials suggests that cardiovascular patients should be screened for hyperhomocysteinemia, when no other risk factor is found. Patients with intermediate/major hyperhomocysteinemia should be properly assessed and treated for vitamin B deficiencies and inherited disorders according to current guidelines. Further trials are needed to assess the lowering homocysteine effect according to hyperhomocysteinemia categories at baseline.PMID:36170884 | DOI:10.1055/a-1952-1946

Essen Stroke Risk Score Predicts Clinical Outcomes in Heart Failure Patients With Preserved Ejection Fraction: Evidence From the TOPCAT trial
This study sought to determine whether the Essen Stroke Risk Score (ESRS) could predict the risks of adjudicated clinical outcomes in patients with Heart failure with preserved ejection fraction (HFpEF) from the TOPCAT trial. Of 3,441 HFpEF patients with a mean follow-up of 3.3 years, the risk of stroke ranged from 0.32% per year at an ESRS of 1-2 points to 1.71% per year at a score of ≥6 points. Each point increase in ESRS was associated with increased risks of stroke (HR=1.33 [95% CI 1.16-1.53]) with a C-index of 0.68, as well as myocardial infarction (HR=1.60 [95% CI 1.40-1.83]; C-index, 0.75), HF hospitalization (HR=...

NO/cGMP/PKG pathway in platelets:the therapeutic potential of PDE5 inhibitors in platelet disorders
J Thromb Haemost. 2022 Aug 11. doi: 10.1111/jth.15844. Online ahead of print.ABSTRACTPlatelets are the "guardians" of the blood circulatory system. At sites of vessel injury, they ensure hemostasis and promote immunity and vessel repair. However, their uncontrolled activation is one of the main drivers of thrombosis. To keep circulating platelets in a quiescent state, the endothelium releases platelet antagonists including nitric oxide (NO) that acts by stimulating the intracellular receptor guanylyl cyclase (GC). The latter produces the second messenger cyclic guanosine-3',5'-monophosphate (cGMP) that inhibits platelet ac...
