Cardiovascular comorbidities in atopic dermatitis: Potential implications for the use of Janus kinase inhibitors
To the Editor: The Food and Drug Administration recently approved the use of 2 oral Janus kinase (JAK) inhibitors —abrocitinib and upadacitinib—for patients with moderate-to-severe atopic dermatitis (AD) not adequately controlled with systemic therapies, including biologics, or when the use of such treatments is inadvisable.1,2 In September 2021, the Food and Drug Administration issued a boxed warning for a ll JAK inhibitors regarding the increased risk of serious infections, heart attack, stroke, cancer, blood clots, and death.

28464 Hidradenitis suppurativa and association with stroke: Systematic review and meta-analysis
Background: Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition characterized by painful nodules, abscesses, sinus tract formation, and scarring. HS has a number of associations with systemic disorders, including cardiovascular disease and metabolic syndrome. However, the relationship between HS and strokes is not well established. To assess this potential association, a systematic review and meta-analysis was performed.

28675 A case of systemic amyloidosis diagnosed by dermatologic consult
A 49-year-old male with PMH of hypertension and diabetes was admitted to the neurology ICU following thrombolytic therapy for left MCA occlusive thrombus. Dermatology was consulted for desquamation of the lower lip, lateral tongue, and bruising of the eyelid and cheeks. Patient originally presented to outside hospital with bilateral lower extremity swelling and shortness of breath. BNP and troponins were elevated, and ACS protocol was initiated. He had stroke-like symptoms while at the outside facility and CT showed MCA occlusive thrombus.

The dream of a good night's sleep for our dermatology patients
Are you getting adequate sleep? Chances are that you are not and the repercussions could be profound. Sleep deprivation has been associated with insulin resistance, diabetes mellitus, obesity, hypertension, obstructive sleep apnea, anxiety and depression, and an increased risk of myocardial infarction and stroke. Quality of life (QoL) may suffer by poor job performance, an increased risk of accidents, and family stress.1

18756 Skin biopsy for the diagnosis of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy: A report of two cases
Skin biopsy has become a useful tool for the diagnosis of extracutaneous diseases as a relatively benign procedure with a good performance. The evaluation of dermal and epidermal innervation and vasculature show distinct changes that support the diagnosis of specific neurologic diseases. Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an inherited angiopathy caused by mutations in the NOTCH3 gene on chromosome 19. It is characterized by stroke and vascular cognitive impairment in the young and other neurological manifestations.

In memoriam: Haines Ely, MD, FAAD
Aside from the news about the canceling of the American Academy of Dermatology Annual Meeting in Denver, there is horrible news that is difficult with every key stroke as I write today …Dr Haines Ely, long-standing pinnacle member of Noah Worcester, Pacific Dermatologic Association, RxDerm, and the American Academy of Dermatology, has passed away after a long battle with prostate cancer. He is survived by his wife, Jenny, and adult daughters, Meredith Bordoni (Michael), Rebecca Ely, and Tess Ely.

The bacteria stroke: Leclercia adecarboxylata folliculitis in a healthy swimmer —an emerging aquatic pathogen?
A 12-year-old healthy boy presented with a 2-month painful acneiform eruption. On exam erythematous follicular papules and pustules were scattered over bilateral shoulders and back. These surfaces were moderately tender to light palpation; the patient even complained that clothing in contact with his skin elicited pain. Doxycycline 75 mg po qd × 10 days was prescribed to cover common Staphylococcus aureus folliculitis. Due to the atypical presentation of significant pain, a bacterial culture was collected.
