Warfarin may reduce risk of ischemic stroke by preventing atrial fibrillation for patients with heart failure and sinus rhythm
As compared with the general population, patients with reduced ejection fraction who are in sinus rhythm are observed with a higher risk for ischemic stroke. This relationship is caused by left ventricular stasis, a systemic hypercoagulable state, and endocardial dysfunction in a situation of impaired left ventricular systolic function . Antithrombotic therapy for atrial fibrillation in patients with acute ischemic stroke or transient ischemic attack
Although warfarin is more effective than are antiplatelet drugs for preventing stroke in patients with atrial fibrillation (AF), it is usually underutilized after stroke for various reasons. A global AF registry study indicated that warfarin was prescribed even less frequently for stroke prevention in Asia than in the West [1]. The major problem of using warfarin in Asian populations is the increased risk of intracranial bleeding. The purpose of this study was to evaluate the use of antithrombotic therapy at discharge in a consecutive cohort of patients with acute ischemic stroke or transient ischemic attack (TIA) and AF.

Non-Vitamin K Oral Anticoagulants (NOACs) for stroke prevention in Asian patients with atrial fibrillation: Time for a reappraisal
Non-Vitamin K Antagonist Oral Anticoagulants (NOACs) have changed the landscape for stroke prevention in atrial fibrillation (AF). Given the huge burden of AF in Asians, more attention to stroke prevention is clearly needed. Aiming to provide an overview and reappraisal of stroke prevention in Asians with AF, we searched MEDLINE for information on NOACs in Asians. In addition, abstracts from national and international cardiovascular meetings were studied to identify unpublished studies.In the 4 recent Phase 3 trials comparing NOACs to warfarin, a consistent pattern is evident.

The impact of inpatient rivaroxaban versus warfarin on hospital-based outcomes when used for stroke prevention in patients with anticoagulant naïve, new-onset nonvalvular atrial fibrillation
Target specific oral anticoagulants (TSOACs) are alternatives to warfarin for stroke prevention in patients with non-valvular atrial fibrillation (NVAF). Stroke prophylaxis in NVAF does not require immediate therapeutic anticoagulation; however, delaying discharge in patients treated with warfarin to achieve a therapeutic international normalized ratio (INR) may prolong hospital length of stay (LOS) compared to use of a TSOAC. We aimed to evaluate patients hospitalized for new-onset, NVAF that were initiated on either rivaroxaban or warfarin for stroke prevention on hospital-based outcomes.

Effect of prior anticoagulation therapy on stroke severity and in-hospital outcomes in patients with acute ischemic stroke and atrial fibrillation
We aimed to assess the prevalence of prior anticoagulation therapy (warfarin or non-vitamin K antagonist oral anticoagulants [NOACs]) among patients with acute ischemic stroke (AIS) and atrial fibrillation (AF) in China and investigate the associations between prior anticoagulation therapy and initial stroke severity and in-hospital outcomes.

Simultaneous onset of myocardial infarction and ischemic stroke in a patient with atrial fibrillation: Multiple territory injury revealed on angiography and magnetic resonance

Fatal stroke following treatment with apixaban in a patient with atrial fibrillation and left atrial appendage thrombus
Atrial fibrillation increases the risk of ischemic stroke 4–5 fold [1] and doubles the mortality independently of other vascular risk factors [2]. The vast majority (>90%) of thrombi associated with atrial fibrillation originate from the left atrial appendage [3]. The direct factor Xa inhibitor apixaban is superior to warfarin in terms of stroke prevention with a lower risk of bleeding and mortality [4].

Renal function, P-gp-affecting drugs and new anticoagulants for stroke prevention
New oral anticoagulants (NOAs), like the thrombin-inhibitor dabigatran etexilate or the factor Xa-inhibitor rivaroxaban showed similar efficacy as the vitamin-K-antagonist (VKA) warfarin for stroke prevention in patients with atrial fibrillation (AF) . Effect of antithrombotic and antiplatelet agents for ischemic stroke in atrial fibrillation patients with dialysis
With great interest, we read Chen et al. study about antithrombotic agents in patients with end stage renal disease (ESRD) and atrial fibrillation (AF) [1]. The authors found that anti-platelet or warfarin treatment could not lower the risk of ischemic stroke in patients with ESRD. We have some concerns about this study.
