Cryptogenic stroke: Contemporary trends, treatments, and outcomes in the United States
Conclusions In a large contemporary nationwide registry, CS accounted for 20% of ISs among patients with a documented stroke etiology. Patients with CS had a distinct profile of treatments and outcomes relative to other subtypes. Improved subtype documentation and further research into CS are warranted to improve care and outcomes for patients with stroke.

Treatment of a hot carotid: More fuel is needed to clarify the best treatments
The majority of patients presenting with a stroke or TIA receive imaging for carotid stenosis. Treatment of symptomatic carotid stenosis is familiar to most neurologists since high-profile clinical trials have been conducted in the past. The most well-known in North America is the North American Symptomatic Carotid Endarterectomy Trial (NASCET).1 NASCET and additional analysis by Rothwell et al.2 led to the recommendation that if the patient is an otherwise suitable candidate for carotid endarterectomy (CEA), then performing the operation within 2 weeks of the last symptomatic event is desirable.

Conclusions Over-the-counter cognitive enhancement supplements may contain multiple unapproved drugs. The health effects of consuming untested combinations of unapproved drugs at unpredictable dosages without clinician oversight in supplements are unknown.

Pediatric Moyamoya Syndrome Secondary to Tuberculous Meningitis: A Case Report
We report the case of a female patient who initially presented at 6 years of age with TBM and developed moyamoya syndrome requiring revascularization surgery. Results She was found to have basilar meningeal enhancement and right basal ganglia infarcts. She was treated with 12 months of antituberculosis therapy and 12 months of enoxaparin and maintained on daily aspirin indefinitely. However, she developed recurrent headaches and transient ischemic attacks and was found to have progressive bilateral moyamoya arteriopathy. At age 11 years, she underwent bilateral pial synangiosis for the treatment of her moyamoya syndrome. ...
