Historical Primer on Obstetrics and Gynecology Health Inequities in America: A Narrative Review of Four Events
Obstet Gynecol. 2023 Oct 1;142(4):779-786. doi: 10.1097/AOG.0000000000005331. Epub 2023 Sep 7.ABSTRACTFour historical events provide context for racial injustices and inequities in medicine in the United States today: the invention of race as a social construct, enslavement in the Americas, the legal doctrine of Partus sequitur ventrem, and the American eugenics movement. This narrative review demonstrates how these race-based systems resulted in stereotypes, myths, and biases against Black individuals that contribute to health inequities today. Education on the effect of slavery in current health care outcomes may prevent...

Disparities in Timeliness of Endometrial Cancer Care: A Scoping Review
CONCLUSIONS: Evidence gaps on disparities in timeliness of endometrial cancer care reveal emphasis of patient-driven help-seeking delays, reliance on health care-derived databases, underutilization of participatory methods, and a paucity of intervention studies.SYSTEMATIC REVIEW REGISTRATION: Given that PROSPERO was not accepting systematic scoping review protocols at the time this study began, this study protocol was shared a priori through Open Science Framework on January 13, 2021 (doi: 10.17605/OSF.IO/V2ZXY), and through peer review publication on April 13, 2021 (doi: https://doi.org/10.1186/s13643-021-01649-x).PMID:37...

Real-World Utilization of an Intrauterine, Vacuum-Induced, Hemorrhage-Control Device
CONCLUSION: For both vaginal and cesarean births in real-world settings, rapid and effective bleeding control was achieved with an FDA-cleared intrauterine vacuum-induced hemorrhage-control device. The safety profile was consistent with that observed in the registrational trial (NCT02883673), and SAEs or adverse device effects were of the nature and severity expected in the setting of PPH. This device is an important new tool for managing a life-threatening condition, and timely utilization may help to improve obstetric hemorrhage outcomes.CLINICAL TRIAL REGISTRATION: ClinicalTrials.gov, NCT04995887.PMID:37713322 | DOI:10....

Primary Human Papillomavirus Testing and Other New Technologies for Cervical Cancer Screening
Obstet Gynecol. 2023 Sep 14. doi: 10.1097/AOG.0000000000005393. Online ahead of print.ABSTRACTCervical cancer screening has saved the lives of millions in regions where routine gynecologic care is readily accessible. As screening continues to evolve away from cervical cytology to primary human papillomavirus (HPV) testing, robust prospective cohort data have allowed for precise risk stratification and improved our ability to identify those at greatest risk of high-grade dysplasia and decrease unnecessary diagnostic procedures. New technologies such as p16/Ki-67 dual stain testing and HPV methylation panels, which offer com...

Care and Monitoring of Pregnant Patients With Left Ventricular Assist Devices
Obstet Gynecol. 2023 Sep 14. doi: 10.1097/AOG.0000000000005351. Online ahead of print.ABSTRACTCardiovascular disease is one of the leading causes of maternal mortality in the United States. Although still rare, pregnancy in patients with left ventricular assist devices (LVADs) is becoming more common. Typical indications for the use of LVADs in reproductive-aged females include ischemic cardiomyopathy, nonischemic (familial) dilated cardiomyopathy, peripartum cardiomyopathy, and some forms of myocarditis. An LVAD drains blood through a cannula placed into the apex of the left ventricle and then returns it to the proximal a...

Data Equity for Asian American and Native Hawaiian and Other Pacific Islander People in Reproductive Health Research
Obstet Gynecol. 2023 Sep 7. doi: 10.1097/AOG.0000000000005340. Online ahead of print.ABSTRACTThough racial and ethnic disparities in sexual and reproductive health outcomes are receiving greater interest and research funding, the experiences of Asian American and Native Hawaiian and Other Pacific Islander (NHPI) people are often combined with those of other racial and ethnic minority groups or excluded from data collection altogether. Such treatment is often rationalized because Asian American and NHPI groups comprise a smaller demographic proportion than other racial or ethnic groups, and the model minority stereotype ass...

Association Among Race, Ethnicity, Insurance Type, and Postpartum Permanent Contraception Fulfillment
CONCLUSION: In unadjusted models, we find marked racial disparities in fulfillment of permanent contraception. Controlling for individual- and facility-level factors eliminated associations among race, ethnicity, insurance type, and fulfillment, likely because covariates are mediators on the pathway between racism and fulfillment.PMID:37678912 | DOI:10.1097/AOG.0000000000005328

Bridging Health Disparities and Improving Reproductive Outcomes With Health Center-Affiliated Doula Programs
Obstet Gynecol. 2023 Sep 7. doi: 10.1097/AOG.0000000000005337. Online ahead of print.ABSTRACTDoulas are community perinatal professionals trained in pregnancy health, childbirth preparation, labor support, lactation counseling, and postnatal care. Doulas serve as patient advocates and provide laboring patients with continuous one-to-one support including informational, physical, and emotional support. Research shows that access to continuous labor support contributes to positive health outcomes such as increasing rates of spontaneous vaginal delivery, shorter labor, less need for analgesics, and increased satisfaction with...

The Power of Language in Hospital Care for Pregnant and Birthing People: A Vision for Change
We describe the use of language in written documentation, verbal communication, and behaviors associated with caring for pregnant people. We also present recommendations for change, including alternative language at the individual, clinician, hospital, health systems, and policy levels. We define birth as the emergence of a new individual from the body of its parent, no matter what intervention or pathology may be involved. Thus, we propose a cultural shift in hospital-based care for birthing people that centers the birthing person and reconceptualizes all births as physiologic events, approached with a spirit of care, par...
