Outcome of surgery for acute infective endocarditis: does preoperative stroke have an impact on mortality?
ConclusionSurgical treatment for IE still remains a challenge with high morbidity and mortality. Patients with preoperative neurologic dysfunction due to septic embolism have good early postoperative results without increased mortality.

Acute cardio-embolic stroke in a preschooler —a surgical dilemma
We present a rare case of acute cardio-embolic stroke in a 3-year-old child due to left atrial myxoma which was successfully treated.

Results of carotid endarterectomy with the use of temporary shunts with reduced retrograde pressure in the internal carotid artery — analysis of the multicenter Russian register
ConclusionTemporary carotid shunts during carotid endarterectomy were associated with increased rates of asymptomatic stroke. There were no statistically significant differences in the incidence of non-fatal or fatal stroke, myocardial infarction, and mortality.

Robot-assisted totally endoscopic coronary bypass surgery
AbstractTotally endoscopic coronary artery bypass (TECAB) surgery can exclusively be undertaken with the aid of operative robots. In the past two decades, surgical remote manipulator systems —predominantly the daVinci® devices—have brought us the reality of endoscopic internal mammary artery harvesting and coronary bypass anastomoses via minimally invasive thoracic port access. Single up to quadruple TECAB interventions are recently feasible; the procedure can be delivered either a s beating heart applying endoscopic vacuum stabilizer or under cardioplegic arrest on heart-lung machine. Significant surgical team learni...

Transcatheter aortic valve replacement in low-risk patients: superiority or shifting goalposts and statistical crystal-gazing?
AbstractTranscatheter aortic valve replacement (TAVR) has been compared to surgical aortic valve replacement (SAVR) in two different industry-sponsored trials (Edwards Lifesciences —the SAPIEN 3 system; Medtronic—the CoreValve, EvolutR, and EvolutPRO) in patients with low risk for surgical aortic valve replacement. In the balloon-expandable SAPIEN 3 system, requirements for both non-inferiority (P <  0.001) and superiority (P <  0.001) were met for the primary outcome which was a composite of death from any cause, stroke, or re-hospitalization. In the self-expanding EVOLUT valve study, the requirement for...

NOBLE trial —is it time to revise the guidelines?
AbstractThe Nordic –Baltic–British left main revascularization trial (NOBLE) is a prospective, randomized, multicentre, non-inferiority trial comparing percutaneous coronary angioplasty (PCI) with coronary artery bypass grafting (CABG) for revascularization of patients with unprotected left main coronary artery (L MCA) stenosis. The primary outcome was a combined endpoint of all-cause mortality, stroke, non-procedural myocardial infarction and repeat revascularization. CABG was found to be superior to PCI with respect to the 5-year MACCE rates (28% vs. 19%) with a hazard ratio (HR) of 1.58 (95% CI 1.24–2.01 ). All-ca...

Novel technique of left atrial appendage occlusion —a pilot study
AbstractA clot in the left atrial appendage (LAA) is an important cause of cardio-embolic stroke. Concomitant occlusion of the LAA during cardiac surgery is found to have reduced postoperative stroke. A study was designed to observe the results of LAA occlusion in 17 patients undergoing coronary artery bypass graft surgery (CABG) and aortic valve replacement (AVR). The LAA was occluded epicardially with a SIRONIX 60-mm linear noncutting stapler (Healthium Medtech Pvt limited, Peenya, Bengaluru). The effectiveness of occlusion was confirmed by trans-esophageal echocardiography and at 2  years of follow-up with trans-thorac...

Minimally invasive aortic valve replacement with sutureless valves
ConclusionsThe combination of minimally invasive AVR and sutureless technology has been associated with excellent results; therefore, SU-AVR might be considered the real “alternative” to TAVI procedure for high-risk patients.

Hybrid aortic arch repair: 10-year experience from India
ConclusionHybrid aortic arch replacement can be performed with good postoperative outcome. Type II hybrid is better than type I hybrid in our experience. As experience increases, the outcome continues to improve.

Early outcomes in patients undergoing off-pump coronary artery bypass grafting
ConclusionEarly outcome of off-pump CABG was excellent in this study. Increased incidence of deep sternal wound infection remains a concern. Multicenter study with a larger sample size is required for a dependable evaluation of the efficacy of off-pump CABG in Indian population.

Aortic arch aneurysms and dissection —open repair is the gold standard
AbstractThe aortic arch repair is one of the most complex surgeries and carries a high risk of complications as well as mortality. Since 1975, when the arch repair was first done by Randall B. Griepp using hypothermic circulatory arrest, many new technologies were introduced. But even with the use of antegrade and retrograde perfusion techniques and improvement of surgical techniques and grafts, the rate of mortality, cerebral, spinal, and visceral damage was much higher as compared to any other cardiac surgeries. With further developments aimed at less invasive approaches, thoracic endovascular aortic repair (TEVAR) along...

Patients who develop post-operative atrial fibrillation have reduced survival after off-pump coronary artery bypass grafting
ConclusionThe incidence of POAF was lower than reported in the wider global literature. Increasing age and development of post-operative sepsis were strong independent predictors of POAF. POAF increases the morbidity; length of hospital stay and these patients show decreased survival after off-pump CABG.

David aortic valve-sparing reimplantation versus biological aortic root replacement: a retrospective analysis of 411 patients
ConclusionsShort-term outcomes for both AVr-D and ARr-B-bio are excellent in patients with aortic root pathology. The long-term outcomes were associated with comparable survival and freedom from reoperation. AVr-D may be preferable to ARr-B-bio in patients with suitable pathoanatomy.

Outcome of CentriMag ™ extracorporeal mechanical circulatory support use in critical cardiogenic shock (INTERMACS 1) patients
ConclusionOur results demonstrate an excellent outcome with the use of the CentriMag ™ device in this seriously ill population. Despite requiring multiple procedures, over 58% of patients were discharged from hospital with 5-year survival of 46%.
