Acute camptocormia as the presenting symptom of tetanus
We report an unusual case of tetanus with camptocormia as the initial symptom. Case presentation A 77-year-old female farmer with unremarkable medical history arrived at our hospital due to difficulty in swallowing and a dropped head which were noticed 4 days prior to the visit. On examination at arrival, she had camptocormia with increased tonus of rectus abdominis muscle. She also had hoarseness in speech and mild limitation in opening her mouth. No wound was observed. Investigation On laryngoscopy, saliva was pooling around the epiglottis with decreased mobility. Therefore, she was admitted to the intensive care un...

Facial onset sensory motor neuronopathy (FOSMN) syndrome: an unusual amyotrophic lateral sclerosis phenotype?
Facial onset sensory and motor neuronopathy (FOSMN) syndrome may represent an amyotrophic lateral sclerosis phenotype as indicated by associated with the heterozygous D90A superoxide dismutase-1 (SOD-1) gene mutation. Facial onset sensory and motor neuronopathy (FOSMN) syndrome is a rare and slowly progressive neurological disorder heralded by development of sensory symptoms within the face (trigeminal nerve distribution), and followed by evolution of sensory and motor deficits in a rostral-caudal direction.1 The motor deficits are characterised by lower motor neurone features including muscle weakness and wasting, cramps ...

The classification of neurological disorders in the 11th revision of the International Classification of Diseases (ICD-11)
Bringing the ICD into the 21st Century. How will ICD-11 reflect the evolution in our understanding of neurological disease over the past twenty years? The WHO, the body responsible for the International Classification of Diseases (ICD) is currently preparing the 11th revision (ICD-11) which is scheduled for approval by the World Health Assembly in 2015. Among its many uses, the ICD is the premier tool for clinical coding and capturing morbidity and mortality statistics worldwide. As the revision process passes the halfway stage, it is timely to consider how the neurology chapter is shaping up. We outline the major changes ...

Usefulness of frataxin immunoassays for the diagnosis of Friedreich ataxia
Conclusions Frataxin measurements from peripheral tissues can be used to identify FRDA patients and carriers. While multiple cell types and assays may be useful for diagnostic purposes, each assay and cell type used has its advantages and disadvantages depending on study design and scope.

Autosomal-recessive complicated spastic paraplegia with a novel lysosomal trafficking regulator gene mutation
Conclusions We diagnosed these patients as having adult CHS presenting spastic paraplegia with cerebellar ataxia and neuropathy. The clinical spectrum of CHS is broader than previously recognised. Adult CHS must be considered in the differential diagnosis of AR-HSP.

Isolated cognitive relapses in multiple sclerosis
Conclusions and relevance We showed that ICRs were not associated with changes in mood, fatigue levels or cognitive performance self-evaluations. Our study introduces an operational definition of ICRs and suggests to their role as a factor for cognitive decline in MS.

Abnormal inflammatory activity returns after natalizumab cessation in multiple sclerosis
Conclusions This study identifies rebound after NTZ cessation as an association of relapses and high MRI activity.

Cortical susceptibility-weighted imaging hypointensity after stroke-like episode in MELAS
A 49-year-old woman with a known m.3243A>G tRNALeu(UUR) mutation (ie, the most frequent mutation in mitochondrial encephalopathy, lactic acidosis and stroke-like episodes (MELAS)) diagnosed in the presence of migraine, diabetes mellitus, neurosensory hearing loss, short stature, cognitive deficit, ataxia and elevated lactate levels, presented with subacute aphasia and right hemiplegia. Brain MRI showed a typical stroke-like lesion in the left temporal and parietal lobe and prerolandic cortex. At this time, gradient-echo T2-weighted imaging showed hyperintensities in the involved regions (also visible on T2-weighted and ...

Multifocal motor neuropathy: lost in conduction block?
How to optimise intravenous immunoglobulin (IVIg) treatment? Should IVIg treatment strategies be tailor-made with the aid of biomarkers to avoid habituation? There has been an exponential increase in the usage of IVIg for an ever-growing number of conditions. It is a product with an excellent safety record without the side effects of steroids or other immunosuppressive agents. Despite the fact that there have been numerous recent advances in studying the mechanisms of action of IVIg in many conditions, there is still much to be learned. IVIg has had a major impact in neurology, haematology, immunology, rheumatology and der...

Tobacco smoking and excess mortality in multiple sclerosis: a cohort study
Conclusions Tobacco smoking can account for some of the excess mortality associated with MS and is a risk determinant for all-cause and MS-related death.

Assessment of cancer risk with {beta}-interferon treatment for multiple sclerosis
Conclusions There was no evidence of an increased cancer risk with exposure to IFNβ over a 12-year observation period. However, the trend towards an association between IFNβ and breast cancer should be investigated further.

Major stressful life events in adulthood and risk of multiple sclerosis
Conclusions Our national cohort study provides little evidence for a causal association between major stressful life events (as exemplified by divorce or the loss of a child or a spouse) and subsequent MS risk.

Brain atrophy and disability progression in multiple sclerosis patients: a 10-year follow-up study
Conclusion This long-term study shows that whole brain, cortical and putamen atrophy occurs throughout the 10-year follow-up of this MS cohort and is more pronounced in the group that showed disability progression at 5, but not at 10 years of follow-up. Overall, GM atrophy showed better association with disease progression than WM atrophy over 5-year and 10-year follow-up.
