Factors effecting natural course of egg allergy
Conclusions. Higher levels of egg sIgE, skin prick test induration and anaphylaxis at onset or during oral food challenge, can give hint about persistence of egg allergy.PMID:36891735 | DOI:10.23822/EurAnnACI.1764-1489.286

Towards a global vision of molecular allergology: a map of exposure to airborne molecular allergens.
Authors: Cecchi L, Dell'albani I, Frati F Abstract Allergy diagnostics have changed in the last 10-15 y, moving from the use of extracts for in vivo and in vitro diagnosis to the Component Resolved Diagnosis, based on purified or recombinant allergens. As expected, aerobiology developed similarly, and measurement of allergens in both outdoor and indoor air is now feasible. With the aim of promoting a global view of molecular allergy, we have drawn a map of exposure to molecular aeroallergens in Italy on the bases of geo-climatic regions, maps of pollen distribution, and published data on the molecular profile of se...

Phleum pratense molecular pattern across Italy.
Conclusion. This study demonstrates that Phleum pratense sensitization is relevant in Italy, but there are significant geographical variations variations as far as pollen exposure and pattern of IgE reactivity to the considered allergen components are concerned. This information may have clinical relevance in managing patients allergic to grass pollen. PMID: 28752721 [PubMed - in process]

Predictor of buckwheat allergy in children based on challenge test results: a retrospective observational study in Japan
Eur Ann Allergy Clin Immunol. 2021 Jul 5. doi: 10.23822/EurAnnACI.1764-1489.224. Online ahead of print.ABSTRACTBuckwheat (BW) is a major food allergen and one of the leading causes of food-induced anaphylaxis in Japan. The standard method of diagnosing food allergy is the oral food challenge (OFC). The BW-specific IgE (BW-sIgE) value is used to assess BW allergy but its utility is limited. The aim of the present study was to identify factors with predictive value for the diagnosis of BW allergy using the OFC.We evaluated 37 patients who were classified into the positive or negative group according to their OFC results. Ten ...

Factors influencing the prescription of allergen immunotherapy: the allergen immunotherapy decision analysis (AIDA) study.
Authors: Frati F, Incorvaia C, Cadario G, Fiocchi A, Senna GE, Rossi O, Romano A, Scala E, Romano C, Ingrassia A, Zambito M, Dell'albani I, Scurati S, Passalacqua G, Canonica GW, AIDA Study Group Abstract The evidence of efficacy of allergen immunotherapy (AIT) for respiratory allergy has been demonstrated by a number of meta-analyses. However, the daily practice of AIT is quite different from controlled trials, facing challenges in terms of selection of patients, practical performance, and, of particular importance, use of allergen extracts of inadequate quality. We here performed a survey, named the Allergen Immu...

Anomalous cutaneous absorption of allergens as cause of skin prick testing adverse reactions in adult patients. Clinical and experimental evidence.
CONCLUSIONS: This study adds clinical and experimental evidences that localized impairments of permeability can occur in adult patients with apparently normal skin. Special precautions should be taken when a change of the drops' normal shape and cohesion is seen, because allergy prick-testing in such areas is potentially associated with increased risk of large local or systemic reactions. Erythema multiforme caused by sildenafil in an HIV(+) subject.
Authors: Pitsios C Abstract Erythema multiforme is mainly caused by drug allergy and infections. This is the case of a HIV-positive, 49-year-old male, recently cured for syphilis, that presented erythema multiforme minor, five days after taking sildenafil. He had a fast recovery, only with the use of antihistamines. Cell-mediated allergy to sildenafil was confirmed six months later, with the use of patch-tests. PMID: 26934741 [PubMed - in process]

Severe anaphylaxis to Propofol: first case of evidence of sensitization to soy oil.
We report a case of anaphylaxis to propofol in an adult patient without food allergy to soy, but with a latent sensitization to soy. An IgE-dependent allergy to propofol was established by a basophil activation test. Here, we document for the first time the existence of specific IgEs to a 65kDa protein, found in soybean oil and soy flour. In the absence of data on the reactogenic threshold for allergenic food proteins injected intravenously, a risk appears to be established and leads us to recommend a systematic detection for proteins in the refined soybean oil used in the pharmaceutical industry for intravenous products. ...

Allergen immunotherapy in polysensitized patient.
The objective we are facing is that in many patients tested as poly-reactive, we have to distinguish in which cases it is a true polysensitization, and when it is due to cross-reactivity of specific IgE antibodies induced by panallergens. This may really determine when AIT may be an appropriate course of action. The article focuses on this problem in more detail, applying the long time Czech and Slovak experience with allergy testing and allergen immunotherapy. Patients with breakthrough reactions to iodinated contrast media have low incidence of positive skin tests.
CONCLUSION: This preliminary finding suggests that patients with prior breakthrough reactions have significantly lower immunologically proven ICM reactions (positive STs) if compared to non-breakthrough patients. According to that, a considerable number of breakthrough reactions seems to be non-allergic hypersensitivity reactions or reactions which could be mostly prevented by a proper, well-timed skin testing. Larger prospective studies are needed to confirm these results, with a more careful analysis of patients' risk factors, a laboratory assessment that includes an in vitro allergy diagnostics, and hopefully a drug pro...

Anaphylaxis to hidden potato allergens in a peach and egg allergic boy.
We report an anaphylactic reaction to candies in an egg and peach allergic boy. We performed a study to identify responsible allergens for the reaction. We investigated hidden egg and peach allergens in the candies, but they were not found. Finally, the causative allergen resulted to be a vegetable protein from potato peel. We diagnosed a new allergy in our patient and Sol t 4 was identified as the responsible allergen of the anaphylactic reaction. We conclude that responsible allergens should always be studied and identified in whatever allergic reaction in order to prevent new reactions. PMID: 28120607 [PubMed - in process]

Letter to the Editor: Can dog allergen immunotherapy reduce concomitant allergic sensitization to other furry animals? A preliminary experience.
We describe a case of dog allergy in which we explored if dog AIT (DAI) could reduce a concomitant allergic sensitization to other allergens of furry animals. Our case demonstrates the efficacy of sublingual DAI on SPTs, symptom score, and spirometric responses despite persistent exposure to dog allergens at home in a patient sensitized, but not exposed, to several other furry animals. Moreover, this is the first report suggesting that DAI is able to reduce SPTs responses not only to dog, but also to other furry animals such as rabbit, horse, mouse, rat, hamster, cow. We recommend an accurate anamnesis and diagnosis of dog...
